目的 探讨二维超声在肾上腺血肿诊断及随访监测中的应用价值.方法 回顾性分析解放军98医院2009年3月至2012年4月期间经CT、MRI证实的55例超声初诊为肾上腺血肿患者的二维超声声像图表现和随访结果.结果 55例中51例超声诊断符合,4例误诊.51例患者中发现53个血肿,51个位于右侧,2个位于左侧.51例患者均采用非手术保守治疗,32例2个月内消失,15例半年内消失,1年后复查均显示肾上腺无异常.结论 对于大部分肾上腺血肿患者,结合外伤史,二维超声可以快速、准确地做出诊断,并可作为有效的随访监测手段.

Objective To investigate the application value of two-dimensional ultrasound in diagnosis and follow-up monitoring of adrenal hematoma.Methods The two-dimensional ultrasonographic features and follow-up results of 55 patients with adrenal hematoma dignosed by two-dimensional ultrasound and confirmed by CT or MRI were analyzed retrospectively from March 2009 to April 2012.Results Among the 55 cases,51 cases were consistented with CT or MRI diagnosis and 4 cases were misdiagnosed.Fifty-three hematomas were found in 51 cases.Fifty-one hematomas were on the right side and 2 hematomas on the left side.All the 51 patients were dealt with non-operation conservative treatment.Thirty-two hematomas disappeared within 2 months and 15 hematomas within half a year.After a year,all injured adrenal showed normal imaging.Conclusions For most patients with adrenal hematoma,two-dimensional ultrasound can quickly,accurately diagnose by combining the history of trauma,and it can serve as an effective means of follow-up monitoring.
