目的 探讨围生期窒息对早产儿自然杀伤(NK)细胞及T淋巴细胞亚群水平变化的影响及其临床意义.方法 采用流式细胞仪对31例轻度窒息早产儿及25例重度窒息早产儿生后2h静脉血T淋巴细胞亚群进行测定,对照组为同期在本科住院的无窒息早产儿30例.结果 重度窒息组CD3-/CD16+ CD56+,CD3+、CD4+、CD4 +/CD8+明显低于对照组与轻度窒息组,差异有统计学意义(P<0.05);轻度窒息组CD3-/CD16+ CD56+,CD3+、CD4+、CD4 +/CD8+亦明显低于对照组,差异有统计学意义(P<0.05);轻重度窒息组各项指标与对照组比较差异无统计学意义,但均高于对照组,P>0.05.结论 窒息缺氧可导致早产儿NK细胞及T淋巴细胞亚群紊乱,随着窒息程度加重,早产儿免疫功能改变愈加显著.

Objective To investigate the effect of asphyxia in perinatal period on NK cell and T-lymphocyte subsets levels in preterm infants.Methods Flow cytometers was used to detect the venous blood T-lymphocyte subsets levels of 31 preterm infants with mild asphyxia and 25 premature infants with severe asphyxia at 2 hours after birth.At the same period,30 preterm infants without asphyxia hospitalized in our department were enrolled as control group.Results Compared with the control group and the mild asphyxia group,CD3-/CD16+ CD56+,CD3+,CD4+,CD4/+ CD8+ of the severe asphyxia group significantly decreased(P < 0.05) ; CD3-/CD16+ CD56+ and CD3+,CD4+,CD4+/CD8+ of the mild asphyxia group were lower than those in the control group,there were significant differences (P < 0.05).There was no significant difference in each indexes between mild and severe asphyxia group and control group,but both of them were higher than that in the control group (P > 0.05).Conclusions Asphyxia can cause disturbance of T-lymphocyte subsets in preterm infants and the more severe of asphyxia,the more dramatic change of NK cell and T-lymphocyte subsets have been found.
