目的 通过测量人工全髋关节术前与术后术侧股骨偏心距,分析重建股骨偏心距的程度与术后双下肢不等长程度的关系.方法 选取接受单侧人工全髋关节初次置换术患者79例做为研究对象.其中男40例,女39例,年龄22～80岁,平均51.78岁.术前、术后拍摄骨盆正位X线片,对股骨偏心距、双下肢的长度差进行测量,研究股骨偏心距的重建与术后双下肢长度差之间的关系.结果 术后双侧股骨偏心距的差值与术后双下肢的长度差有明显相关性(r =0.632,P＜0.05);股骨偏心距重建组的等长率与未重建组之间差异有统计学意义(x2=24.012,P＜0.05);在股骨偏心距得到重建的患者中,双侧股骨偏心距差值较小组与差值较大组的术后双下肢等长率之间差异有统计学意义(P＜0.05).结论 为了避免术后双下肢不等长的发生,全髋关节置换术中应重建股骨偏心距,而且在重建偏心距的范围内也应尽量做到双侧股骨偏心距相等.

목적 통과측량인공전관관절술전여술후술측고골편심거,분석중건고골편심거적정도여술후쌍하지불등장정도적관계.방법 선취접수단측인공전관관절초차치환술환자79례주위연구대상.기중남40례,녀39례,년령22～80세,평균51.78세.술전、술후박섭골분정위X선편,대고골편심거、쌍하지적장도차진행측량,연구고골편심거적중건여술후쌍하지장도차지간적관계.결과 술후쌍측고골편심거적차치여술후쌍하지적장도차유명현상관성(r =0.632,P＜0.05);고골편심거중건조적등장솔여미중건조지간차이유통계학의의(x2=24.012,P＜0.05);재고골편심거득도중건적환자중,쌍측고골편심거차치교소조여차치교대조적술후쌍하지등장솔지간차이유통계학의의(P＜0.05).결론 위료피면술후쌍하지불등장적발생,전관관절치환술중응중건고골편심거,이차재중건편심거적범위내야응진량주도쌍측고골편심거상등.
Objective By measuring the femoral offset (FO) before and after total hip arthroplasty (THA),the relationship between reconstruction of femoral offset and the leg length discrepancy after surgery was analyzed.Methods Seventy-nine patients who had accepted unilateral primary total hip arthroplasty were chose as the study object,including 40 males and 39 females,aged 20 ～ 80 years,averaged 51.78 years.The pelvis X-ray was taken to measure the leg length and femoral offset before and after surgery,the relationship between the reconstruction of the femoral offset and leg length discrepancy was studied.Results The difference of the femoral offset between the two sides was negatively correlated with the leg length discrepancy after operation (r =0.434,P =0.03).There was significant difference in the rate of equal leg length between the reconstruction of femoral offset group and the un-reconstruction of femoral offset group(x2 =4.13,P ＜0.05).In the reconstruction of femoral offset group,there was significant difference in the postoperative rate of leg length equality between the small bilateral FO difference group and big bilateral FO difference group (P ＜ 0.05).Conclusions In order to avoid double leg length discrepancy,femoral offset should be rebuilt during THA surgery,and in the scope of the reconstruction of femoral offset,equal bilateral femoral offset should try to be reconstructed.
