目的探讨采用吻合器痔上黏膜环切术与传统痔手术对中老年糖尿病患者的影响.方法随机选取鹤壁煤业集团公司总医院2010年3月至2013年9月收治的行痔手术的中老年糖尿病患者90例,将其随机平均分配为试验组和对照组,其中试验组采用吻合器痔上黏膜环切术(PPH),对照组实施传统外膜内扎手术,对比两组患者治疗前后的血糖变化、胰岛素及胰岛素敏感性变化情况,并对结果进行分析.结果试验组术后血糖水平明显低于对照组,差异有统计学意义(P＜0.05);试验组术后胰岛素敏感性明显优于对照组,差异有统计学意义(P＜0.05).结论采用吻合器痔上黏膜环切术对中老年糖尿病患者进行治疗,有助于降低手术对患者血糖水平和胰岛素抵抗的影响,建议在临床上进一步推广.

Objective To investigate the effects of stapler hemorrhoidal mucosa circumcision and traditional hemorrhoid surgery on elderly patients with diabetes.Methods From March 2010 and September 2013,90 elderly patients with diabetes underwent hemorrhoid surgery in our hospital were randomly divided into experiment group and control group.The patients in experimental group were treated with the mucosal ring hemorrhoid stapler resection(PPH),and the patients in control group were treated with traditional tie surgery.The blood glucose,insulin and insulin sensitivity changes before and after treatment were compared and analyzed between the two groups.Results The blood glucose levels in the experimental group were significantly lower than those in the control group,the difference was significant (P ＜ 0.05); the postoperative insulin sensitivity in the test group was better than that in the control group,the difference was significant (P ＜ 0.05).Conclusions Stapling hemorrhoidal mucosa circumcision for the treatment of elderly patients with diabetes,helps to reduce the impact of surgery on blood glucose levels and insulin resistance,it is recommended to be further promoted in clinical practice.
