目的 研究静脉连用左西孟旦对晚期心衰患者左室功能、心肌重构和心脏生化标志物的影响.方法 选取50例晚期心衰患者,在常规治疗的基础上静脉连用左西孟旦注射液治疗.通过测定每搏心输出量(SV)、心脏指数(CI)、左室射血分数(LVEF)、左室舒张末容积(LVEDV)等的变化,比较用药前后患者左室功能的改善、心肌重构的变化,以及对心脏生化标志物如心肌肌钙蛋白I(cTnI)、脑钠肽(BNP)、基质金属蛋白酶-9(MMP-9)等的影响.结果 左西孟旦静脉连用后治疗组患者呼吸困难及全身临床症状缓解,左室功能提升,SV、CI、LVEF均上升(P均<0.05),LVEDV及cTnI、BNP、MMP-9等的表达水平也不同程度地下降(P均<0.05).结论 左西孟旦连续用药能显著改善晚期心衰患者的左室功能,在一定程度上延缓了心肌重构的进展,并对心脏生化标志物产生了重大影响.

목적 연구정맥련용좌서맹단대만기심쇠환자좌실공능、심기중구화심장생화표지물적영향.방법 선취50례만기심쇠환자,재상규치료적기출상정맥련용좌서맹단주사액치료.통과측정매박심수출량(SV)、심장지수(CI)、좌실사혈분수(LVEF)、좌실서장말용적(LVEDV)등적변화,비교용약전후환자좌실공능적개선、심기중구적변화,이급대심장생화표지물여심기기개단백I(cTnI)、뇌납태(BNP)、기질금속단백매-9(MMP-9)등적영향.결과 좌서맹단정맥련용후치료조환자호흡곤난급전신림상증상완해,좌실공능제승,SV、CI、LVEF균상승(P균<0.05),LVEDV급cTnI、BNP、MMP-9등적표체수평야불동정도지하강(P균<0.05).결론 좌서맹단련속용약능현저개선만기심쇠환자적좌실공능,재일정정도상연완료심기중구적진전,병대심장생화표지물산생료중대영향.
Objective To study the effects of Levosimendan by continuous applications on left ventricular function,myocardial remodeling and cardiac biochemical markers in patients with terminal heart failure.Methods Fifty patients with advanced heart failure were chose.The patients were given levosimendan consecutively on the basis of conventional treatments.By measuring the cardiac output stroke volume (SV),cardiac index (CI),left ventricular ejection fraction (LVEF) and left ventricular end-diastolic volume (LVEDV) before and after treatment,the improvement of left ventricular function,the changes of myocardial remodeling,as well as the expression of the cardiac biochemical markers such as cTnI,BNP,MMP-9 were compared.Results Mter continous application of leosimendan,dyspnea and systemic clinical symptoms were relieved,left ventricular function were improved,SV,CI and LVEF were increased (P < 0.05),and LVEDV as well as expression levels of cTnI,BNP,MMP-9 also were differently decreased (P < 0.05).Conclusions Levosimendan can obviously improve left ventricular function in patients with terminal heart failure,extently delay the progress of myocardial reconstruction,and has significant impact on cardiac biochemical markers.
