目的探讨纳洛酮联合无创正压通气治疗COPD并发II型呼吸衰竭和肺性脑病的临床效果.方法将86例患者分成观察组和对照组,两组均抗炎,吸氧,解痉平喘,止咳化痰及营养支持等综合治疗.观察组再给予纳洛酮及无创正压通气治疗,比较两组疗效.结果观察组和治疗组总有效率为93%、65.1%,差异有显著性(P<0.05).血气分析观察组治疗后血PaO2、SpO2较治疗前有明显的增高(P<0.05),PaCO2明显下降(P<0.05),治疗后观察组PaO2、SpO2、PaCO2较对照组差异有统计学意义(P<0.05).结论纳洛酮联合无创正压通气治疗COPD并发II型呼吸衰竭和肺性脑病的临床疗效显著.

@@@@Objective To investigate the effect of naloxone combined with noninvasive positive pressure ventilation in the treatment of COPD complicated with II type respiratory failure and pulmonary encephalopathy clinical effect. Methods 86 patients were divided into observation group and control group,two groups of anti-inflammatory,oxygen inhalation,antispas-modic,relieving cough and eliminating phlegm and nutritional support treatment. The observation group to give naloxone and noninvasive positive pressure ventilation treatment,compared the effect of two groups. Results in the observation group and treatment group total effective rate was 93%,65.1%,there was significant difference(P<0.05 ). Blood gas analysis of obser-vation group after treatment of blood PaO2,SpO2 markedly increased(P<0.05 ),PaCO2 decreased significantly(P<0.05 ), after treatment were observed in group PaO2,SpO2,PaCO2 than the control group and the difference was statistically significant(P<0.05). Conclusion naloxone combined with non-invasive positive pressure ventilation in the treatment of COPD complicated with II type respiratory failure and pulmonary encephalopathy clinical observation.
