目的探讨切开复位锁定钢板内固定治疗跟骨 SandersⅢ、Ⅳ型骨折的方法和疗效.方法自2008年1月~2011年2月期间收治29例31足SandersⅢ、Ⅳ型跟骨关节内骨折,均采用切开复位锁定钢板内固定治疗,应用影像学检查及美国足踝骨科协会(AOFAS)后足评分系统进行结果评价.结果所有病例随访12~38个月,平均18.3个月.术前测量B hler角0.7°±8.8°,Gissane角101.9°±6.4°,跟骨高度(丘部)28.7±5.3mm,跟骨宽度36.7±3.0mm,术后1年后测量B hler角为28.9°±6.1°,Gissane角119.9°±4.3°,跟骨高度(丘部)40.3±3.5 mm,跟骨宽度31.1±1.8mm,术前、术后对比各项差别均有统计学意义(<0.01).按AOFAS评分系统评价术后足部功能,平均82.6分,优良率77.5%.结论对于跟骨SandersⅢ、Ⅳ型骨折采用切开复位锁定钢板内固定治疗能够取得良好疗效.

@@@@Objective Investigate the methods and clinical outcome of Sanders typeⅢ~Ⅳcalcaneal fractures treated with locking plate. Methods From January 2008 to Feburary 2011, open reduction and internal fixation with calcaneal locking plate were performed in 31 cases of Sanders typeⅢ-Ⅳcalcaneal fracture, and the clinical outcome were retrospectively analysised. Results Pre-operation: B hler angle was 0.7°±8.8°, Gissane angle was 101.9°±6.4°, and the calcaneous width was 36.7±3.0mm; post-opration: B hler angle was 28.9°±6.1°, Gissane angle was 119.9°±4.3°, calcaneous height was 28.7±5.3mm, and the calcaneous width was 31.1±1.8mm. Significant differences was found ( <0.01). According to the American Orthepaedic Foot and Ankle Society hindfoot score, the rate of excellent and good was 77.5%. Conclusions Relatively Satisfactory effect could be achieved with locking plate fixation in Sanders typeⅢ~Ⅳcalcaneal fractures.
