目的以往研究表明,不同造影剂的使用量在造影剂肾病的发生中有着重要意义.本研究旨在分析比较不同使用量的碘普罗胺对于患者肾功能的损害情况.方法将252名脑血管病患者根据造影剂的使用量分为AB两组:A组≤150ml,B组>151ml.通过测量术前及术后24-48h的肌酐、肌酐清除率、尿素氮、尿酸以及胱抑素C等来观察肾功能的变化.结果 A组中cCr、BUN、UA以及B组中cCr、UA在造影术后的测定值与术前有统计学差异.11人sCr上升>基础值的25%,其中A组3例, B组8例,CIN发生率分别为2%和7.4%.结论使用造影剂对肾功能有一定的影响,并且大剂量造影剂的使用可以提高CIN的发病率,但是在56-245ml的剂量范围内与造影剂的使用量没有明显的相关性.

Objectives Previous studies have shown that different contrast volume(CV) is important in generating contrast-induced nephropathy(CIN). The aim of our research was to analyze weather various CV would lead to diverse condition in renal function. Methods A total of 252 patients, with acute cerebrovascular disease, were enrolled. According to their CV, we divided them into two groups, people with CV≤150ml in group A, and CV>151ml in B. Then we can evaluate these patients' renal function through testing their serum Cr(sCr), creatinine clearance rate(cCr), urea nitrogen(BUN), uric acid(UA) and cystatin-C(CysC) level. Results After the digital subtraction angiography(DSA), both of the cCr, BUN and UA in group A and cCr and UA in group B were statistical significance. Moreover, the sCr of 11 people, 3 cases in A and 8 cases in B, have increase by 25% from baseline. Conclusions Contrast medium would have certain influence on renal function. Besides, more volume would increase the morbidity of CIN. However, there was no clear correlation ship between them when the CV ranges from 56ml to 245ml.
