目的:比较下斜肌转位术治疗先天性麻痹性斜视在位垂直性斜视的术前后的立体视功能的变化.方法:采用同视机图片与Titmus立体视图本,对27例先天性的麻痹性斜视,做手术前正前方与代偿头位方位的、与近距离立体视定性和定量测定.实行手术后,再同样做与近立体视锐度测定,进行分级对比分析.结果:所有病例术后代偿头位基本消除或改善,视力提高2~5行.立体视从无到有,视锐度从800"~60"逐级提高由周边的体视转变为黄斑体视、以至中心凹体视.结论:先天性麻痹性斜视立体视觉术前正前方可有部分发育,代偿头位方向好正前方向,经下斜肌完全转位术后,不仅解决了代偿头位、视力问题,而且能使近立体视均得到不同程度的提高.

목적:비교하사기전위술치료선천성마비성사시재위수직성사시적술전후적입체시공능적변화.방법:채용동시궤도편여Titmus입체시도본,대27례선천성적마비성사시,주수술전정전방여대상두위방위적、여근거리입체시정성화정량측정.실행수술후,재동양주여근입체시예도측정,진행분급대비분석.결과:소유병례술후대상두위기본소제혹개선,시력제고2~5행.입체시종무도유,시예도종800"~60"축급제고유주변적체시전변위황반체시、이지중심요체시.결론:선천성마비성사시입체시각술전정전방가유부분발육,대상두위방향호정전방향,경하사기완전전위술후,불부해결료대상두위、시력문제,이차능사근입체시균득도불동정도적제고.
Objective: To investigate the variation of three-dimensional visual function before and after vertical heterotropia operation by inferior obliquus version. Methods:27 patients of congenital paralytic squint were qualitatively and quantitatively detected their distant and near three-dimensional visual acuity between front aspect and compensative aspect before operation by synoptophore picture and Titmus three-dimensional visual function picture. After operation, the distant and near three-dimensional visual acuity were detected and compared. Results:The compensative head aspects of all the patients were eliminated or improved. Sight improved by 2~5 lines. Three-dimensional visual function appeared. Visual acuity elevated from 800’’ to 60’’. Circumjacent stereopsis turned into limbus luteus retinae stereopsis and even fovea centralis stereopsis. Conclusion:Inferior obliquus version was significantly effective on improving distant and near three-dimensional vision.
