目的比较妇科腹腔镜手术采用单管喉罩与双管喉罩全身麻醉对呼吸循环系统的影响.方法选取妇科腹腔镜择期手术120例,ASAⅠ~Ⅱ级,随机分为单管喉罩组(D组)和双管喉罩组(S组).记录两组插喉罩前后、气腹后、体位改变(头低脚高20度)后、拔喉罩前后的呼吸循环变化情况.结果两组在插喉罩前后、气腹后、拔喉罩前后的呼吸循环变化比较无显著性差异(P>0.05),但单管喉罩组(D组)在体位改变后呼吸影响较大,两组比较有显著性差异(P<0.05).结论单管喉罩在体位改变(头低脚高20度)后比较容易出现漏气,潮气量降低,而双管喉罩则无此情况发生,在妇科腹腔镜手术中,使用双管喉罩比单管喉罩安全性更高.

Objective Comparison of gynecological laparoscopic operation using a single laryngeal mask airway and proseal laryngeal mask airway under general anesthesia on respiratory and circulatory system effects.Methods Selection of elective operation in 120 cases of gynecological laparoscopy,ASAⅠ~Ⅱlevel,Randomly divided into single laryngeal mask airway(Team D) and proseal laryngeal mask airway (Team S)group.In two group were recorded before and after the insertion of laryngeal mask,after pnenmoperitoneum,after changing of position (Low head feet high 20 degrees),pul the laryngeal mask airway and respiratory and circulatory changes. Results In two group before and after inserting laryngeal mask, after pnenmoperitoneum,pul the laryngeal mask airway and respiratory and circulatory changes in comparison with no significant difference(P>0.05),But the single group laryngeal mask airway(Team D) in postural changes after respiratory influence,two groups had significant difference(P<0.05).Conclusion Single tube and laryngeal mask airway in after changing of position more prone to leakage,tidal volume reduction,while the proseal laryngeal mask airway does not happen,in gynecological laparoscopic operation,using the proseal laryngeal mask airway than single laryngeal mask airway high security.
