目的:探讨尺骨鹰嘴截骨垂直双钢板内固定治疗青壮年C型肱骨髁间骨折临床疗效.方法:回顾性分析笔者自2008年10月~2012年5月收治的得到随访的13例C型肱骨髁间骨折的临床资料,男8例,女5例,年龄22~53岁,平均43.5岁,骨折类型按AO/ASIF分类为:C1型1例,C2型6例,C3型6例,全部采用经尺骨鹰嘴截骨垂直双钢板内固定治疗.结果:术后随访6个月~38个月,平均14个月,最后1次随访时骨折及截骨处均全部骨性愈合.根据Mayo法评分为65~100分,平均84.23分,其中优(90~100分)5,良(75~89分)6例,差(小于60分)2例,优良率为84.6%.结论:经尺骨鹰嘴截骨显露充分,垂直双钢板内固定牢靠,允许早期进行肘关节功能锻炼,可获得满意的临床疗效,是目前手术治疗青壮年C型肱骨髁间骨折的较好术式.

Objective:To evaluate the clinical outcome of olecranon osteotomy approach and perpendicular double plates fixation for type C intercondylar fracture of distal humerus in young adults. Methods:Fifteen patients with AO/ASIF type C fractures of the distal humerus(1 eases type C1, 6 cases type C2, and 6 cases type C3),who were admitted into our Hospital from Oct 2008 to May 2012 and followed up, were treated with olecranon osteotomy approach and perpendicular double plates fixation by the author. There were 8 males and 5 females. The mean age was 43.5 years with a range from 22 to 53 years. Results:All the patients were followed up from 6 to 38 months with the average of 14 months and all the osteotomies and fractures healed in the last following-up. According to Mayo Elbow Performance Scores, excellent(90-100) in 5, good(75-89) in 6, poor(<60) in 2. The excellent and good rate was 84.6%. Conclusion: The olecranon osteotomy approach and perpendicular double plates fixation might be one of the good choices for surgical treatment of type C intercondylar fractures in young adult distal humerus, since it could provide excellent exposure and solid fixation, and permit early rehabilitation, and obtain satisfactory clinical outcomes.
