目的:观察使用踝足矫形器对脑卒中偏瘫患者足底生物力学特征的影响.方法:将56例脑卒中偏瘫患者随机分为对照组和使用踝足矫形器组,每组各28例,2组患者均接受相同的常规康复治疗,矫形器组在治疗中站立和行走训练时使用踝足矫形器,每天间断穿戴支具不少于4小时,分别于治疗前和治疗4周后对2组患者足下垂生物力学特征进行评定.结果:矫形器组患者经4周治疗后,其足下垂生物力学特征均较治疗前有非常显著的改善(P<0.01),对照组较治疗前显著改善(P<0.05);矫形器组的生物力学特征改善优于对照组(P<0.05).结论:踝足矫形器对改善偏瘫患者足下垂具有很好的作用.足底生物力学研究对于指导康复治疗方案的制定、疗效的评估具有理论及现实指导意义.

Objective:To observed the effects of using the ankle-foot orthosis biomechanical characteristics of stroke patient with hemiplegia plantar. Methods: 56patients with hemiplegic stroke patients were randomly divided into observation group and the control group,28cases in each group. Both two groups received the same conventional rehabilitation. The observation group received ankle-foot orthosis based on standing and walking training everyday with a continuous wear brace for a period of not less then 4h, two groups of patients was foot drop biomechanical characteristics were evaluated before treatment and after 4 weeks of treatment. Results: After 4 weeks treatment of the observation group patients the foot drop biomechanical characteristics had more significantly improvement then prior treatment(P<0.01) and the control group had little improvement then prior treatment(P<0.05). The observation group biomechanical characteristic improved superior to control group(P<0.05). Conclusion: ankle-foot orthosis to improve hemiplegic stroke patients with foot drop could have good role. The foot biomechanical research had theoretical and practical significance for guiding rehabilitation program development and evaluation of the efficacy.
