目的:应用关节镜下钻孔减压治疗膝关节地图型骨挫伤,评价其短期随访的疗效.方法:2006年11月至2009年6月之间收治的13例膝关节地图型骨挫伤患者,女8例,男5例.年龄平均42.8岁.所有患者均有外伤史,主诉膝关节疼痛,MRI 检查证实为地图型骨挫伤.所有患者均在关节镜下行钻孔减压术,术中用2mm 克氏针使用电钻在骨挫伤部位的负重关节软骨面以外区域进行钻孔.结果:术后3个月患者的 VAS 疼痛评分、Lysholm 评分和影像学分级明显改善,术后6个月有进一步改善.结论:关节镜下钻孔减压治疗膝关节地图型骨挫伤的短期随访结果是良好的,可以作为骨挫伤的治疗方法之一.

Objective:To assess the efficacy of short-term folow-up of geographic bone bruises in knee which treated by arthroscopic driling decompression. Methods: From November 2006 to June 2009, 13 cases of geographic bone bruises in knee were involved, which included 8 females and 5 males with the average age of 42.8 years. Al patients had history of trauma, and complained of knee pain. MRI examination confirmed the diagnosis of geographic bone bruises. Al patients were accepted arthroscopic driling decompression. During the operation 2mm Kirschner wires were used to dril the parts of the articular cartilage surface of bone bruises, which is outside the region of weight-bearing parts. Results: The VAS pain score, Lysholm score and radiographic grade improved significantly 3 months after operation. Further improvement was found 6 months after operation. Conclusion: There is a good short-term folow-up result of arthroscopic driling decompression in the treatment of geographic bone bruises in knee. It is one of the suitable methods for bone bruises.
