目前国内临床路径仍存在电子化程度不高,适应性差,与医院信息系统知识共享性不足等问题.在深入了解医院临床路径实际运行环境的基础上,抽取关键领域知识,并对其进行规范化表达,提出了一种基于本体的临床路径知识库设计与实现方案;以肺叶切除术为例,建立其临床路径知识库,并实现知识库与电子病历(EMR)的集成;根据患者需求生成个性化诊疗计划方案,灵活处理由于患者病情指标改变或诊疗条件变化而引起的变异,从而有效地解决上述问题.临床路径本体知识库的建立为共享临床路径知识,规范诊疗行为提供了标准机制,为新一代智能化临床信息系统的发展奠定了基础.

Nowadays domestic clinical pathways (CP) are still faced with the problems of immature electronization, lack of adaptability and insufficient knowledge sharing with hospital information systems. Based on in-depth understanding of CP execution environment, this paper extracts essential domain knowledge and formalizes it into a standardized expression, an ontology-based design and implementation scheme of CP knowledge base is proposed;a Lobectomia Pulmonalis CP knowledge base is built as an example, and further integration with electronic medical records (EMR) is realized;according to the needs of patients, a personalized diagnostic and treatment plan is generated with the flexibility to deal with the variation caused by changes of patients' clinical status or other care conditions in the healthcare settings, thus effectively solve the above mentioned problems. The establishment of ontology-based clinical pathway knowledge base provides a standard mechanism to share the knowledge involved in clinical pathways and normalize treatment activities, thus paves the way for the development of a new generation of intelligent clinical information systems.
