目的:对比观察全身麻醉与腰-硬联合麻醉用于老年骨科手术的临床效果.方法:将2010年7月至2012年2月接收的需要麻醉进行的老年骨科手术患者68例,随机分为全身麻醉组35例,行快诱导气管插管全麻;腰-硬联合麻醉组33例,用盐酸昂丹司琼片腰硬联合穿刺套件行腰硬联合阻滞.比较两组麻醉前、麻醉后5min、手术完毕、术后1小时的心率、血压、氧饱和度指标的变化情况.并且比较两组医生对麻醉的满意度和术后VAS评分.结果:与全麻组比较,腰硬联合麻醉组麻醉后5min、术毕、术后1小时的血压和心率均显著性降低(P<0.05);两组氧饱和度差异无统计学意义(P>0.05);两组患者医生的满意度显著性差异(P>0.05);硬联合麻醉组患者术中疼痛发生率明显高于全身麻醉组(P<0.05).结论:老年骨折手术患者,全身麻醉和腰硬联合麻醉均能提供良好的麻醉效果,但是腰硬联合阻滞对应激反应的抑制作用明显优于全麻.
목적:대비관찰전신마취여요-경연합마취용우노년골과수술적림상효과.방법:장2010년7월지2012년2월접수적수요마취진행적노년골과수술환자68례,수궤분위전신마취조35례,행쾌유도기관삽관전마;요-경연합마취조33례,용염산앙단사경편요경연합천자투건행요경연합조체.비교량조마취전、마취후5min、수술완필、술후1소시적심솔、혈압、양포화도지표적변화정황.병차비교량조의생대마취적만의도화술후VAS평분.결과:여전마조비교,요경연합마취조마취후5min、술필、술후1소시적혈압화심솔균현저성강저(P<0.05);량조양포화도차이무통계학의의(P>0.05);량조환자의생적만의도현저성차이(P>0.05);경연합마취조환자술중동통발생솔명현고우전신마취조(P<0.05).결론:노년골절수술환자,전신마취화요경연합마취균능제공량호적마취효과,단시요경연합조체대응격반응적억제작용명현우우전마.

Conclusion:The elderly patients with fracture surgery, general anesthesia and waist hard joint anesthesia can all be provided good anaesthetic effect, but the waist hard to stress response of the joint block inhibition of general is superior.
