目的:探讨主动脉腔内支架隔绝术治疗降主动脉瘤的可行性及疗效.方法:2009年2月~2013年1月,对52例 stanfordB型主动脉夹层、5例真性动脉瘤、3例假性动脉瘤、15例主动脉壁间血肿合并主动脉穿透性溃疡行主动脉腔内隔绝术.在进行腔内修复治疗前再血管化2例,其中右颈总动脉—左颈总动脉旁路转流1例,无名动脉、左颈总动脉与升主动脉建立旁路1例,19例腔内隔绝直接封堵左锁骨下动脉,2例患者使用“烟囱”技术在左锁骨下动脉植入支架.结果:75例手术均获成功,2例 I 型内漏在近端添加 cuff 后纠正,有2例 II 型内漏.69例随访,随访1~35个月,平均(24.3±10.7)个月.1例 II 型内漏3个月后消失.结论:主动脉腔内支架隔绝术疗效可靠,创伤小,操作简单,并发症少.大多数降主动脉病变可采用腔内支架隔绝术治疗.

Objective:To investigate the indications and efficacy of endovascular stenting for the treatment of Descending aortic aneurysm.Methods From February 2009 to January 2013,52 patients with Stanford type B aortic dissection,5 cases of true aneurysm, 3 cases of pseudoaneurysm,and 15patients with intramural hematoma complicated with ulcer of the aortic wall were treated by endovagcular stenting in our hospital.Hybrid procedures included 1 right common carotid artery(RCCA)-LCCA bypass,1 innominate artery- left common carotid artery - ascending aorta bypass.Left subclavian artery was totally occluded in 19 patients,and 2 chimney stems were deployed. Results The operation was completed successfully in all of the 75cases.2cases had intraopcrative type I endoleaks ,Cuff extension were used for treating type I endoleaks. 2cases had intraopcrative type II endoleaks. A follow up of (24.3± 10.7)months (range1-35months)was carry out in 69 patients.1 case type II endoleaks closed spontaneously after three months of follow-up. Conclusions Endovagcular stenting is a simple,safe and effective method for Descending aortic aneurysm.
