目的探讨新诊断2型糖尿病(T2DM)患者血清视黄醇结合蛋白4(RBP4)及骨钙素(BGP)水平,并分析其影响因素,胰岛素抵抗(IR)之间的关系.方法选取新诊断T2DM患者40例(T2DM组)及健康体检者40例(NC组).检测两组空腹血清RBP4和BGP水平,并计算其胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HOMA-β)、胰岛素敏感性(ISI),对RBP4、BGP及其他因素进行单因素相关分析,然后进行多元逐步回归分析.结果(1)T2DM组患者血清RBP4水平明显高于NC组(P<0.01),而T2DM组患者血清BGP水平明显低于NC组(P<0.01)差异有统计学意义.(2)单因素相关分析显示,血清RBP4与体重指数(BMI)、三酰甘油(TG)、总胆固醇(TC)、空腹血糖(FPG)、HOMA-IR呈正相关(P<0.05);与HOMA-β、ISI呈负相关(P<0.01);而血清BGP与BMI、TG、TC、FPG、HOMA-IR呈负相关(P<0.05);与HOMA-β、ISI呈正相关(P<0.01);BGP与RBP4呈负相关(P<0.01).(3)糖尿病血清钙(Ca)、磷(P)水平与对照组比较无显著差异.(4)多元回归分析:BMI和FPG为影响RBP4的最显著因素;HOMA-IR、RBP4为影响BGP的最显著因素.RBP4作为独立危险因素影响血浆BGP水平(r=-0.524,P<0.01).结论 RBP4、HOMA-IR为BGP独立负性影响因素, BGP减少可能与RBP4水平升高所致IR有关.BGP是通过成骨细胞表面的FINS受体来参与IR.

Objective To explore the relationship of serum retinol-binding protein 4 (RBP4)to Bone glaprotein(BGP) expressionand to insulin resistance (IR) in newly-diagnosed type 2 diabetesmellitus (T2DM) patients. Methods Both Newly diagnosed type 2 diabetes patients (T2DM, n=40) and healthy subjects (NC, n=40) were enrolled grouping this study, with their serum levels of RBP4 and BGP measured.Also, insulin resistance index (HOMA-IR) and insulin sensitivity (ISI) were calculated in both group,univariate analysis was performed on RBP4, BGP and other factors, and thenmultiple re-gression analysis .Results (1)Serum level of RBP4 in the T2DM group was significantly higher than in the controlgroup (P<0.01).The level of BGP in the T2DM group was significantly lower than in control (P<0.01). (2)Single-factor correlation analysis showed that ser-um RBP4 was positively correlatedwith BMI,triglyceride(TG),cholesterol(TC),HOMA-IR(P<0.05),negatively cor-relatedwith ISI and HOMA-β(P<0.01). BGP was negatively correlatedwith BMI, TG, TC,HOMA-IR(P<0.05), positively with ISI and HOMA-β(P<0.01),and RBP4 negatively BGP with (P<0.01). (3)In type 2 diabetesmellitus patientsserum Ca and P were normal . (4)Multiple regression analysis discoveryed that BMI and FPG were themost signifi-cant factors influencing RBP4, HOMA-IR RBP4 were the most significant factors influencing BGP. RBP4 was independent risk factor of elevated BGP level. Conclusion: RBP4, HOMA-IR was BGP independent negative influence factors, reduction of BGP may be associated with elevated levels of RBP4 induced by IR. BGP is through the osteoblast surface FINS receptor in IR.
