目的:观察胰岛素注射联合口服格列齐特片治疗糖尿病肾病(DN)的临床疗效及不良反应.方法:选取62例T2DM合并DN患者,按治疗方案不同分为对照组,30例,仅以胰岛素注射;研究组,32例,胰岛素注射联合格列齐特口服.治疗12周后,分别比较两组的空腹血糖(FBG)、餐后2 h血糖(PBG)、糖化血红蛋白(HbA1c)和肾功能指标及不良反应发生率差异.结果:与治疗前比较,治疗后两组FBG、PBG、HbA1c及肾功能指标(BUN、SCr、24 h尿蛋白定量)均有下降(P<0.05);与对照组比较,研究组的上述指标下降程度更明显(P<0.05).两组的低血糖、胃肠道反应及血细胞减少等发生率无差异(P>0.05).结论:胰岛素注射联合格列齐特口服治疗DN的临床疗效显著且不良反应小.

Objective:To observe the curative effects and adverse reactions of insulin injection combination with gliclazide in the treatment of diabetic nephropathy(DN).Method:A total of 62 patients with type 2 diabete mellitus(T2DM)and DN were taken prospective analyses and divided into two groups,control group(30 cases,received with insulin injection)and research group(30 cases,received with insulin injection combined with gliclazide,32 cases),according to therapeutic regimen.The therapeutic courses of the two groups were 12 weeks.The indexes of fasting blood glucose(FBG), 2-hr plasma glucose(PBG),glycolated hemoglobin(HbA1c)and renal function were compared between two groups.The incidence rates of side effects in two groups were observed and recorded.Result:Compared to pre-treatment,the indexes of FBG,PBG,HbA1c and renal function(BUN,SCr and 24-hr urinary protein quantitative)in post-treatment were lower(P<0.05).Compared to control group,the above indexes in research group were lower (P<0.05).But the incidence rates of side effects in two groups were no different,such as hypoglycemia,gastrointestinal reaction,hemocytopenia(P>0.05). Conclusion:The curative effect of insulin injection combination with gliclazide in the treatment of DN is perfect and no obvious side effect.
