目的:探讨强制性使用运动疗法(CIMT)对恢复中风偏瘫患者上肢运动功能的临床疗效.方法:选取我院2011年7月~2012年7月期间收治的符合CIMT适应症的50例中风偏瘫患者,在患者知情许可的情况下,随机平均分为CIMT治疗组和常规康复治疗组.CIMT组采用强制性使用运动疗法4周,RRT组采用以Bobath技术为主的常规康复治疗4周,所有病人均采用上肢机能检查(STEF)评价患者的上肢运动功能和Barth-el指数(BI)评定上肢灵活性和日常生活活动能力,对比两组的临床疗效.结果:经过4周的治疗后,同一组别间治疗前后比较STEF、BI评分有明显提高,具有统计学意义(P<0.05),且CIMT组的各项评分均比常规康复组有显著升高(P<0.05).结论:CIMT疗法对于恢复中风偏瘫患者上肢功能较以Bobath技术为主的常规康复治疗更为有效,能够明显提高偏瘫患者上肢灵活性,提高日常生活能力,值得临床推广.

Objective:To explore the efficacy of constraint induced movement therapy (CIMT) on recovering the upper limb function of apoplectic hemiplegia. Methods:50 patients with apoplec-tic hemiplegia who conformed to the CIMT indication from July 2011 to July 2012. In the patients’ informed and permission, they all were randomly average divided into treatment group of CIMT and routine rehabilitation therapy group. CIMT group were mandatory used the CIMT for 4 weeks, RRT group were used routine rehabilitation therapy which was based on the Bobath technology for 4 weeks. All patients were evaluated the upper limb movement function and Barthel index by the STEF to assess the upper limb flexibility and activities of daily living. Compared the clinic of effica-cy of two groups. Results:After 4 weeds treatment, the STEF.BI more significantly improved in the same group than before treatment. It had statistical significance (P<0.05). Moreover, the CIMT group more significantly improve than the RRT group (P<0.05). Conclusion:The CIMT in recover-ing the upper limb function of apoplectic hemiplegia has more efficacy than the routine rehabilita-tion therapy which is basic on the Bobath technology. It can significantly improve the upper limb flexibility of hemiplegia patient, and improve the ability of daily life. It is worthy to popularize in clinic.
