目的评价羊膜移植术对角膜溃疡的治疗效果.方法选择2009~2011年收治角膜溃疡患者共92例(98眼),其中,细菌性角膜溃疡32例(32眼),真菌性角膜溃疡18例(18眼),病毒性角膜溃疡14例(14眼),外伤(化学性烧伤及烫伤)引起的角膜溃疡28例(34眼).患者经药物治疗效果不佳后行羊膜移植术,随访3~12个月.结果98眼角膜溃疡中91眼治愈,术后1个月左右角膜上皮开始愈合,留下不同程度疤痕.13眼出现羊膜移植片脱落或溶解,其中6眼经再次移植后角膜溃疡愈合,7眼由于病情较重,多次移植后无效.所有羊膜移植患者均未出明显的急性排斥反应.患者术后视力较术前明显好转.结论羊膜移植术在治疗角膜溃疡中具有较好疗效.
목적평개양막이식술대각막궤양적치료효과.방법선택2009~2011년수치각막궤양환자공92례(98안),기중,세균성각막궤양32례(32안),진균성각막궤양18례(18안),병독성각막궤양14례(14안),외상(화학성소상급탕상)인기적각막궤양28례(34안).환자경약물치료효과불가후행양막이식술,수방3~12개월.결과98안각막궤양중91안치유,술후1개월좌우각막상피개시유합,류하불동정도파흔.13안출현양막이식편탈락혹용해,기중6안경재차이식후각막궤양유합,7안유우병정교중,다차이식후무효.소유양막이식환자균미출명현적급성배척반응.환자술후시력교술전명현호전.결론양막이식술재치료각막궤양중구유교호료효.
Objective To evaluate the therapeutic effects of amniotic membrane transplantation(AMT) in corneal ulcer. Methods To collect 92 patients (98 eyes) with corneal ulcer in my hospital from 2009~2011,including bacterial corneal ulcer 32 cases (32 eyes), fungal corneal ulcer 18 cases (18 eyes), viral corneal ulcer 14 cases (14 eye), corneal ulcer caused by trauma(chemical burns and burns) 28 cases (34 eyes). All the patients were treated by AMT after drug treatment which had no efficacy, and followed up for 3 to 12 months. Results 91 eyes healed after amniotic membrane transplantation, the corneal epithelium began to heal within one month after postoperation and remained inordinately scar. The amniotic membrane appeared to be falling off or dissolved in 13 eyes, 6 eyes of that were healed with repeatedly transplantation and 7 eyes failed after more than once transplantation due to serious illness. No acute rejection was observed after AMT in all patients. Conclusion Amniotic membrane transplantation is effective method to treat corneal ulcer.
