[目的]探讨胰肠吻合术后胰瘘的预防方法.[方法]2011年3~12月对28例患者行胰肠吻合时,先用4‐0 vicryl 线作2针“U 形”缝合,将胰腺残端套入空肠内约2 cm ,后用4‐0 Prolene 线连续缝合空肠断端及胰腺上下缘组织,作荷包捆绑固定.[结果]全组吻合时间平均(13±1.5) min ,术后出现 A 级胰漏1例,未发生 B 、C 级胰漏,无吻合口出血.[结论]双“U”套入荷包捆绑式的胰肠吻合术,操作简便、能有效减少胰漏发生率,是对胰肠吻合术的一种安全而有效的改进.
[목적]탐토이장문합술후이루적예방방법.[방법]2011년3~12월대28례환자행이장문합시,선용4‐0 vicryl 선작2침“U 형”봉합,장이선잔단투입공장내약2 cm ,후용4‐0 Prolene 선련속봉합공장단단급이선상하연조직,작하포곤방고정.[결과]전조문합시간평균(13±1.5) min ,술후출현 A 급이루1례,미발생 B 、C 급이루,무문합구출혈.[결론]쌍“U”투입하포곤방식적이장문합술,조작간편、능유효감소이루발생솔,시대이장문합술적일충안전이유효적개진.
Objective]To explore the method for the prevention of pancreatic leakage after pancreaticojejunosto ‐my .[Methods]A total of 28 patients underwent pancreaticojejunostomy from March 2011 to Dec .2011 .Vicryl silk 4‐0 was used for double‐"U" suture and the pancreas was embedded into jejunum about 2cm ,and then prolene silk 4‐0 was used for pursestring suture and bind of jejunum and pancreas .[Results]The average anastomosis time was (13 ± 1 .5)min .Pancreatic leakage of grade A after the operation occurred in a case .No pancreatic leakage of grade B and C occurred .No anastomotic stoma bleeding was found .[Conclusion] Double‐"U" embedding ,pursestring suture and bind style pancreaticojejunostomy is simple and can effectively decrease the incidence of pancreatic leakage ,so it is a safe and feasible improvement method of pancreaticojejunostomy .
