[目的]研究慢性心力衰竭(CHF)患者血清脂联素(APN )与 N 末端 B 型脑钠钛前体(NT‐proBNP)水平的变化及相关性,探讨其与心功能不全严重程度的关系.[方法]选取 CHF 患者63例,根据1994年纽约心脏病协会心功能分级标准(NYHA)分为Ⅱ级17例,Ⅲ级28例,Ⅳ级18例,另选取同期健康体检者37例作为对照组.分别检测其身高、体重、血脂、APN 、NT‐proBNP ,计算体重指数,并对数据进行统计学分析.[结果]CHF 患者血清 APN 和 NT‐proBNP 水平明显高于健康对照组,且随着 NYHA 分级的增加而明显升高,心功能Ⅱ级、Ⅲ级、Ⅳ级组间比较差异均有显著性( P <0.01);CHF 患者治疗前与治疗后血清 APN 、NT‐proBNP 水平比较差异有显著性( P <0.01);对数转换的血清 APN 水平与血清 NT‐proBNP 呈明显正相关(r =0.711, P <0.01).[结论]联合监测血清 APN 和 NT‐proBNP 水平对于判断 CHF 患者的病情及预后具有重要的临床应用价值.

[목적]연구만성심력쇠갈(CHF)환자혈청지련소(APN )여 N 말단 B 형뇌납태전체(NT‐proBNP)수평적변화급상관성,탐토기여심공능불전엄중정도적관계.[방법]선취 CHF 환자63례,근거1994년뉴약심장병협회심공능분급표준(NYHA)분위Ⅱ급17례,Ⅲ급28례,Ⅳ급18례,령선취동기건강체검자37례작위대조조.분별검측기신고、체중、혈지、APN 、NT‐proBNP ,계산체중지수,병대수거진행통계학분석.[결과]CHF 환자혈청 APN 화 NT‐proBNP 수평명현고우건강대조조,차수착 NYHA 분급적증가이명현승고,심공능Ⅱ급、Ⅲ급、Ⅳ급조간비교차이균유현저성( P <0.01);CHF 환자치료전여치료후혈청 APN 、NT‐proBNP 수평비교차이유현저성( P <0.01);대수전환적혈청 APN 수평여혈청 NT‐proBNP 정명현정상관(r =0.711, P <0.01).[결론]연합감측혈청 APN 화 NT‐proBNP 수평대우판단 CHF 환자적병정급예후구유중요적림상응용개치.
Objective] To investigate the change of serum adiponectin(APN ) and N‐terminal pro‐brain natri‐uretic peptide(NT‐proBNP) in patients with chronic heart failure (CHF) and their correlation ,and to explore the relationship of APN and NT‐proBNP with the severity of cardiac dysfunction .[Methods]A total of 63 patients with heart failure were selected .According to 1994 New York heart association heart function classification stand‐ard(NYHA ) ,17 patients were grade Ⅱ ,28 patients were grade Ⅲ and 18 patients were grade Ⅳ .Other 37 healthy people were selected as control group .The height ,weight ,blood fat ,APN and NT‐proBNP were meas‐ured .Body mass index(BMI) was calculated .The data was analyzed by statistics .[Results] Serum levels of APN and NT‐proBNP in CHF patients were significantly higher than those in healthy control group ,and increased with the NYHA ,and there were significant differences among cardiac function with grade Ⅱ , Ⅲ and Ⅳ ( P < 0 .01) . There were significant differences in serum levels of APN and NT‐proBNP in CHF patients between before and af‐ter treatment( P < 0 .01) .Serum level of APN after logarithmic transformation had obviously positive correlation with NT‐proBNP(r = 0 .711 ,P < 0 .01) .[Conclusion] The combined detection of serum APN and NT‐proBNP in CHF patients has very important value of clinical application for assessing the status and prognosis of patients .
