目的观察盐酸右美托咪定用于临床麻醉中的效果,确定有效、安全的麻醉方案.方法将120例进行全身麻醉的肺叶切除手术患者随机分为A组和B组.A组选用生理盐水、咪达唑仑、维库溴铵、舒芬太尼、丙泊酚;B组选用盐酸右美托咪定、咪达唑仑、维库溴铵、舒芬太尼、丙泊酚.分别记录麻醉药物用量、时间指标及各时间点患者的血流动力学情况.结果右美托咪定有利于插管和拔管的平稳;患者丙泊酚的用量B组小于A组,差异有统计学意义;B组相对于A组具有较短的苏醒时间、拔管时间、自主呼吸恢复时间,差异不具有统计学意义.结论麻醉过程中加用盐酸右美托咪定,可明显削弱麻醉插管的应激反应,降低丙泊酚的用量,保持血流动力学的稳定性,显著提高患者满意度.

Objective To observe and compare the effectiveness analysis on addition of dexmedetomidine hydrochloride to general anesthesia program,which given a reference for choosing an effective and safe anesthesia program. Methods 120 cases of pulmonary lobectomy operation under general anesthesia were randomly divided into A group and B group. A group used saline, midazolam, propofol, sufentanil, vecuronium bromide; B group used dexmedetomidine hydrochloride, midazolam, propofol, sufentanil, vecuronium bromide. The hemodynamic situation of anesthetic drug dosage, time and each time point of patient were recorded.Results Dexmedetomidine hydrochloride insure the stability of intubation tube and tube drawing,the dosage of propofol for group B is less than that of group A, and similar as the revival,extubation and spontaneous recovery time.Conclusion From the perspective of patients,the anesthesia program addition to dexmedetomidine hydrochloride could significantly reduce the need handling adverse reaction rates and the amount of propofol,it also could reduce the stress response during intubation,improve patient satisfaction after surgery and maintain hemodynamic stability.
