目的:总结多孔钽棒支撑植入在成人早期股骨头坏死治疗中的早期临床疗效.方法:2008年1月~2011年12月我院收治的成人股骨头缺血坏死早期患者(均为FicatⅠ、Ⅱ期)15例(18髋),男10例12髋,女5例6髋,26~55岁,平均38.6岁.均采用多孔钽棒支撑植入治疗.观察手术操作过程、及术后早期恢复情况,并在术前术后进行Harris 评分.结果:所有患者均获得随访8~32个月,平均18个月,患者术后疼痛及功能受限症状均明显缓解.至末次随访,术后6个月及术后1年Harris评分较术前均有不同程度的增加.统计结果显示差异均有统计学意义.讨论:多孔钽棒具有良好的生物相容性,手术操作简单、微创,对股骨头及软骨下骨板可提供安全、有效的力学支撑,多孔钽棒支撑植入术方法治疗股骨头坏死可明显改善 Harris评分.早期的临床效果令人满意.

Objective:Summary of porous tantalum rod implants in the adult avascular necrosis of the femoral head in the treatment of early clinical outcomes. Methods:From Jan 2008 to Dec 2011,15 patients(18 hips) with avascular necrosis of femoral head in adult patients at early stage were treated(Ficat type Ⅰ、Ⅱ). There were 10 males(12 hips) and 5 females(6 hips) with an age range of 26-55 years (38.6 years on average).All patients were treated by porous tantalum rod.Observed the operation process and early postoperative recovery,all patients were assessed with Harris score at the preoperative and postoperative. Results:All patients were followed up for 8~ 32 months(An average of 18 months). Patients with postoperative pain and functional limitations were significantly relieved.To the last follow-up, 6 months after the operation and after 1 year compared to preoperative Harris score increase in varying degrees.The statistical results showed that there was significant difference. Conclusion:Porous tantalum rod has good biological compatibility.The operation is simple, minimally invasive.It can provide safe, effective mechanical support for the femoral head and subchondral bone plate.Porous tantalum rod implantation method in the treatment of avascular necrosis of the femoral head can obviously improve the score of Harris.Early clinical effect is satisfactory.
