目的探讨应用镶嵌式外固定支架治疗胫骨干骨不连与骨缺损的临床效果.方法回顾性分析了2008年10月~2011年2月我院对收治的26例胫骨干骨不连与骨缺损患者采用镶嵌式外固定支架治疗的资料.结果26例术后随访12~24个月.单纯加压固定22例,骨不连断端加压固定及同期肢体延长4例.感染性骨不连同时皮瓣修复4例.所有骨不连患者均愈合,平均愈合时间8个月.肢体短缩2cM 以下者2例,骨折端成角2例,针孔感染2例.相邻膝、踝关节功能恢复良好.结论镶嵌式外固定支架治疗胫骨干骨不连及骨缺损方法简单、有效,可早期功能锻炼,感染患者可获一期愈合.

Objective to explore the application of mosaic external fixation in the treatment of tibial shaft nonunion and bone defect. Methods a retrospective analysis of 2008 october to 2011 february in our hospital 26 cases of tibial shaft nonunion and bone defect patients using mosaic external fixation in the treatment of data. Results 26 cases were followed up for 12 ~24 months. Pressurized fixation 22 cases,nonunion of fracture end fixed and the same limb lengthening in 4 cases. infected nonunion of the flap repair in 4 cases. all patients with nonunions healed,the average healing time of 8 months. limb shorten-ing below 2cM in 2 cases,2 cases of fracture angulation,pin tract infection in 2 cases. adjacent the knee,ankle joint function recovered well. Conclusion using mosaic external fixator for treatment of tibia bone nonunion and bone defect of method is simple,effective,early exercise,infection can be a healing period.
