目的观察降糖调脂灵对2型糖尿病大鼠糖、脂代谢的调节及对血清TNF-α和主动脉壁超微结构的影响.方法将40只健康成年雄性Wistar大鼠随机分为4组:对照组;模型组;双胍治疗组;降糖调脂灵治疗组.3个月后生化检测血糖、血脂各项指标;计算胰岛素敏感指数(ISI);放射免疫法检测血清TNF-α水平;透射电镜观察主动脉平滑肌超微结构改变.结果①模型组BS,GHbA1c,INS,TC,TG,LDL水平均明显高于对照组,而ISI,HDL明显低于对照组(P<0.05);双胍治疗组BS,GHbA1c水平明显低于模型组(P<0.05),血清INS,ISI,TC,TG,LDL,HDL水平与模型组比较,无显著性差异(P>0.05);降糖调脂灵治疗组BS, GHbA1c,TC,TG,LDL水平低于模型组(P<0.05),ISI,HDL明显高于模型组和双胍组(P<0.05).②模型组TNF-α值较对照组明显升高(P<0.05);模型组和双胍组TNF-α值无显著性差异(P>0.05);降糖调脂灵治疗组TNF-α值明显低于模型组(P<0.05).③模型组可见主动脉壁平滑肌细胞成退行性改变,核内电子密度升高,异染色质增多,胞质内大量线粒体出现明显的嵴断裂、空泡样变、髓鞘样变,高尔基复合体内出现大空泡.降糖调脂灵治疗组主动脉壁平滑肌细胞损坏轻微,但也有少量平滑肌细胞的线粒体、高尔基复合体和粗面内质网呈现退行性改变,可见部分空泡变、髓鞘样变线粒体和空泡变的粗面内质网.结论降糖调脂灵具有降低TC,TG,LDL,升高HDL的作用;降糖调脂灵具有明显降低血糖、减低胰岛素抵抗、增强胰岛素敏感性的作用;降糖调脂灵的降糖作用虽然略低于二甲双胍,但其减低胰岛素抵抗、增强胰岛素敏感性的作用却明显优于二甲双胍;降糖调脂合剂能够保护动脉壁平滑肌细胞,可能通过减少TNF-α的表达来阻止早期动脉壁硬化的发生和发展.

Obective To observe the regulation of Jiangtang tiaozhi ling on blood sugar (BS),lipid metabolism and effects on TNF-αand ultramicrostructural of aorta wall in type 2 diabetes mellitus(T2 DM) rats.Methods Forty healthy male rats were randomly divid-ed into 4 groups:control group,model group;metformin treatment group;treatment of Jiangtang tiaozhi ling group .After 3 months,serum levels of fasting blood sugar,GHbA1c,insulin(INS),total cholesterol(TC),triglyceride(TG),high density lipoprotein(HDL) and low density lipo-protein(LDL) were checked,insulin sensitivity index(ISI) was calculated;Tumor Necrosis Factor-α(TNF-α) were checked and at the same time,using electron microscope to observe the ultramicrostructural of aorta wall .Results ①The levels of BS,GHbA1c,INS,TC,TG and LDL in Model group were obviously increased than Control group (P<0.05),but ISI and HDL were obviously lower than Control group;the levels of BS,GHbA1c in Metformin trentmrnt group was obviously lower than Model group (P<0.05),but there were no obvious difference between the levels of secrumINS,ISI,TC,TG,LDL,HDL of Model group and that of Metformin group .The level of BS,GHbA1c,TC,TG,LDL in Jian-gtang tiaozhi ling group lower obviously (P<0.05),compared with Model group,but that was higher than Metformin trentment group .The lev-el of INS in Chinese traditionnal medicine group decreased obviously than Model group (P<0.05),but the level of ISI,HDL was higher than Model and Metformin trentment group (P<0.05).②The level of plasma TNF-αin Model group was higher than Normal group (P<0.05);There were no obvious difference about the level of TNF-αbetween Model and Metformin trentment groups;Compared with Model group ,the level of TNF-αin JiangtangTiaozhi Ling group were decreasing (P<0.05).③Electron microscope observation showed that ,there were retro-grade changes in smooth muscle cells in rats aorta wall in Model group ,electronic density and heterochromatin in nucleolus were increasing , most mitochondria were so greatly damaged that a lot of vacuoles appeared ,and there were some big vacuoles in Golgi complex .The smooth muscle cells of rats,aorta wall in Chinese traditional medicine group were slightly damaged ,there were retrograde changes in mitochondria , Golgi complex and rough endoplasmic reticulum in a few smooth muscle cells ,some vacuoles appeared in mitochondria and rough endoplasmic reticulum.Conclusion The Jiangtang TiaoZhi Ling had the effects of decreasing TC ,TG,LDL and increasing HDL on T 2 DM rats;The effects of decreasing blood sugar;Though the effect of The Jiangtang TiaoZhi Ling is inferior to the metformin ,but The Jiangtang TiaoZhi Ling could reduce insulin resistance(IR) and raise insulin sensitivity.Jiangtang tiaozhi ling could protect endothelial cells in aorta wall ,maybe through decreasing the expression of TNF-α,this may be one of the mechanisms of Jiangtang tiaozhi ling to prevent the occurrence and development of atherosclerosis(AS)in the early period of T2 DM.
