目的:探讨血液灌流联用血液滤过治疗急性重度有机磷农药中毒的临床治疗效果.方法:在内科常规综合治疗基础上,对治疗组21例重度急性有机磷农药中毒患者给予血液灌流联用血液滤过(HP+CVVH)治疗,对照组21例采用HP治疗,比较两组患者的昏迷时间、胆碱酯酶(ChE)活性恢复至60%的时间、住院天数、C反应蛋白浓度,以及两组患者治愈率与死亡率.结果:治疗组昏迷时间、ChE活力恢复至60%的时间明显缩短,住院天数显著减少(P<0.05), C反应蛋白浓度下降快(P<0.05),对照组患者1例死于中枢性呼吸衰竭,治疗组都治愈,无死亡病例发生.结论:HP+CVVH治疗急性重度有机磷农药中毒疗效显著,为一种有效的救治重度有机磷农药中毒的方法.

Objective:To investigate the curative effect of hemoperfusion(HP) combined with continuous veno-venous hemofiltration (CVVH) to patients with severe acute organophosphorus insecticide poisoning(AOPP). Methods:All patients with severe AOPP were treated with routine therapy. 21 cases in test group were treated with HP combined with CVVH, 21 cases in control group were treated with HP only. Regaining consciousness、cholinesterase(ChE) activities returning to 60%normal levels、the number of days of hospitalization and concentration of C-reactive protein(CRP) were compared in the two groups. Result:The time of patients regaining consciousness, ChE activities returning to 60%normal levels and hospitalization were shorter in test group than those in control group(P<0.05) , Concentration of CRP decreased faster in test group than those in control group(P<0.05) . 1 patient died of central respiratory failure in control group and no deaths occurred in test group. Conclusions:HP combined with CVVH is effective to severe AOPP and should be a choice to treat this kind of patients.
