目的观察局麻下精索内静脉高位结扎术治疗精索静脉曲张的疗效,为门诊手术治疗精索静脉曲张提供有效方法.方法在局麻下,采用左侧髂前上棘与耻骨结节连线中点,腹股沟韧带上方2 cm处作皮肤切口,经腹膜后高位结扎精索静脉.治疗Ⅲ度原发性精索静脉曲张36例.结果麻醉效果满意.36例手术均成功,其中3例术后阴囊轻度水肿,2~4 d后水肿消退.随访1年无复发,无睾丸萎缩、慢性精索疼痛等并发症.结论局麻下精索内静脉高位结扎术简便,易于操作,恢复快,手术费用低,疗效满意,适于门诊手术.

Objective To observe the curative effects of adopting high ligation in spermatic vein under local anesthesia to treat varico-cele.Methods Under local anesthesia,using the midpoint of the left anterior superior iliac spine and pubic tubercle,the skin incision was done at 2 cm above the inguinal ligament,then the retroperitoneal high ligation of the spermatic vein was conducted so as to treat 36 cas-es with Ⅲ -level primary varicocele.Results The anesthetic effect was satisfactory.36 operations were successful,among which three cases had postoperative scrotal mild edema,but 2 ~ 4 days later the edema disappeared.After one year of follow-up visit,there was not recur-rence,testicular atrophy,chronic spermatic cord pain or other complications.Conclusion High ligation in spermatic vein under local anes-thesia is simple and easy to operate with quick recovery,low-cost surgery,and satisfactory results,which is suitable for clinic surgery.
