目的分析慢性前列腺炎(Chronic prostatitis, CP)患者前列腺液标本中的免疫细胞构成特点.方法2010年12月至2011年4月门诊就诊的慢性前列腺炎患者125例,取同期20例医学院学生前列腺液标本作为正常对照组,采用流式细胞术分析前列腺液中CD4+,CD8+,CD3/CD56(NK),CD14+和CD15+细胞计数及相对比例.结果在Ⅱ型,Ⅲ型CP及正常对照组中,CD4+细胞的检出比例分别为43.2%(16/37),53.4%(47/88)和30%(6/20)(P>0.05);CD15+细胞绝对计数中位值分别为4.6×106/ml,4.6×105/ml 和5.1×104/ml(P<0.001);相对比例为65.3%,46.5%和22.4%(P<0.001);CD14+细胞绝对计数中位值分别为1.3×105/ml,4.7×104/ml 和4.2×103/ml(P<0.001);相对比例为12.4%,5.4%和2.0%(P<0.001);CD8+细胞仅在8.2%(12/145)的样本中检出; CD3/CD56(NK)细胞仅在5.5%(8/145)的样本中检出.结论 CD15+细胞、CD14+细胞在不同类型CP患者和正常人群之间在数量和构成比上存在明显差异;CD4+细胞在Ⅲ型中最常存在,Ⅱ型次之,正常标本中相对较少.CD15+细胞、CD14+细胞可望成为CP分型的分子标志并为未来进一步药物干预提供潜在的靶标.

Objective To explore the characteristics of immune cell compositions in the expressed prostatic secretion (EPS) of chronic prostatitis (CP) patients. Methods The EPS of 125 clinical diagnosed CP patients and 20 healthy volunteers were obtained for routine EPS test, bacterial culture and flow cytometry analysis. The cell count and percentage of CD 4+, CD8+, CD3/CD56(NK), CD14+and CD15+cells were determined with immunolaleling and flow cytometry techniques. Results CD4+cell was present in 43.2%(16/37) of type Ⅱ, 53.4%(47/88) of type ⅢCP and 30%(6/20) of normal control (P>0.05). The median CD15+ cell count was 4.6 × 10 6/ml, 4.6 × 10 5/ml and 5.1×104/ml respectively in typeⅡ, TypeⅢCP and control group (P<0.001). The median percentage of CD15+cell in EPS was 65.3%, 46.5%and 22.4%respectively in type Ⅱ, Type ⅢCP and control group (P<0.001). The median CD14+cell count was 1.3 × 105/ml, 4.7 × 104/ml and 4.2 × 103/ml respectively in type Ⅱ, Type ⅢCP and control group (P<0.001). The median percentage of CD14+cell in EPS was 12.4%, 5.4%and 2.0%respectively in type Ⅱ, TypeⅢCP and control group (P<0.001). CD8+cell was present in 8.2%(12/145) samples only, and the positive rate of CD3/CD56 (NK) cell was only 5.5%(8/145). Conclusion CP patients have elevated levels of CD 15+and CD14+cells in the EPS, especially typeⅡCP patients, which implies the important roles of immunological response in CP mediated by specific immune cells. The significance of CD4+, CD8+and CD3/CD56(NK) cells is unclear. Analysis of immune cells in EPS may improve further molecular classification and target therapy of CP.
