目的:为合理测量医疗产出、制订合理收费标准、有效控制医疗费用上涨等提供科学依据.方法:在陕西省全病种分组(CSAP-DRGs)的基础上,采用绝对值法、相对权重系数法、权重基准利率法3种方法对住院费用标准进行测算和确定.结果:(1)绝对值法,预测出了2011年和2012年的住院费用标准;(2)相对权重系数法确定肺炎和流行性感冒为基准组合,计算出了各个DRG组合的相对权重系数;(3)权重、基准利率法计算出了各个DRG组的权重.结论:以CSAP-DRGs为基础来确定住院费用标准是科学合理的,但需要根据实际情况进行调整.

Objective: To provide scientific evidences for measuring medical outputs rationally, guiding hospitals make reasonable tuition standards, and controlling the rise of medical expenses effectively. Methods: Absolute value, relative weight coefficient and weight benchmark interest rate were used to calculate and determine the hospitalization cost standard based on All Patients DRGs in Shaanxi Province of China (CSAP-DRGs) . Results: (1) Absolute value method predicted the hospitalization cost standard of 2011 and 2012; (2) Relative weight coefficient method determined pneumonia and influenza as the benchmark portfolio and calculated the coefficient of each DRG combination; ( 3) Weight benchmark interest rate method calculated the weight of each DRG group. Conclusion: It is scientific and reasonable to determine the hospitalization cost standard on the basis of CSAP-DRGs, but it needs to be adjusted according to practical situation.
