目的:深入分析和研究超声弹性成像在乳腺小实性病灶中的诊断价值,提高疾病的治疗水平.方法:收集2010年3月-2011年5月在笔者所在医院进行手术治疗的490例女性患者共570个乳腺病灶采用超声弹性成像技术进行检查.将490例女性患者按照病灶的大小分为Ⅰ组和Ⅱ组.其中Ⅰ组共有120例患者和150个病灶,在所有病灶当中最大直径≤10 mm.Ⅱ组共有370例患者和420个病灶,在所有病灶当中最大直径>10 mm.Ⅰ组和Ⅱ组均使用日立EUB8500彩超仪进行超声弹性成像检查,采用弹性成像技术进行评分,分数值在4分及其以上分数为为恶性,分数值在3分及以下分数为良性.以手术检查数据结果为对比数据,比较两组在病变的敏感性、特异性和准确性上的差别.结果:Ⅰ组的120例患者和150个病灶当中敏感性为91.0%、特异性为99.5%、准确性为98.6%.Ⅱ组的370例患者和420个病灶当中敏感性81.6%、特异性96.5%、准确性92.8%.两组在超声弹性成像的敏感性、特异性和准确性上比较差异具有统计学意义(P<0.05).结论:超声弹性成像在乳腺小实性病灶中具有良好的诊断价值,在临床中可以广泛推广.

@@@@Objective:The value of in-depth analysis and study of ultrasonic elastography in diagnosis of small breast solid lesions of,improve the level of treatment of disease.Method:From 2010 March to 2011 May operation treatment of 490 cases of female patients with a total of 570 breast lesions were examined by ultrasound elasticity imaging technology in our institute.490 cases of female patients according to the size of the lesion were divided into group I and groupⅡ.In the I group there are 120 patients and 150 lesions in all cases,the largest diameter≤10 mm.GroupⅡconsists of 370 patients and 420 lesions,in all lesions,maximum diameter of>10 mm.Group I and groupⅡwere using HITACHI EUB8500 color Doppler ultrasound ultrasound elasticity imaging,were scored using elasticity imaging techniques,numerical in 4 points and above fraction of malignant,score at or below 3 scores for benign.The operation inspection data results for comparison of data between the two groups in the lesions,the sensitivity,specificity and accuracy of the difference.Result:Of 120 patients in I group and 150 lesions,the sensitivity was 91%,specificity was 99.5%,the accuracy was 98.6%.370 patients in groupⅡand 420 lesions sensitivity of 81.6%,specificity of 96.5%,accuracy of 92.8%.Two groups of sensitivity,in ultrasound elasticity imaging specificity and accuracy has statistical significance(P<0.05).Conclusion:Ultrasound elasticity imaging has good diagnostic value in small breast solid lesions,can be widely applied in clinical practice.
