目的评价血友病性关节病的 MRI 表现及其评分与出血和病程的关系.资料与方法选取行 MRI 检查的36例血友病性关节病患者的65个关节,其中膝关节40个,肘关节4个,踝关节11个,髋关节10个.对所有关节的 MRI表现进行 Denver 评分,分析其与关节出血及病程的相关性.结果血友病性关节病的 MRI 表现为关节积液或积血,滑膜增生伴含铁血黄素沉积等,严重者见软骨下侵蚀、囊变或软骨缺失等.Denver 评分显示65个关节中0分10个,1~6分14个,7~10分41个.Denver 评分与病程和总出血次数呈显著正相关(r =0.672、0.770, P<0.05).结论 MRI 能够明确显示血友病性关节病的异常表现,且其评分与病程和关节总出血次数具有显著相关性.

Purpose To evaluate the MRI appearance of hemophilic arthropathy and the correlation of Denver score with haemorrhage and the course of disease. Materials and Methods MR images of 36 patients with 65 joints hemophilia athropathy, including 40 knee joints, 4 elbow joints, 11 ankle joints and 10 hip joints, were retrospectively analyzed. The MRI appearance of all joints was evaluated according to Denver score. The correlation of MRI appearance with haemorrhage and the course of disease was analyzed. Results MRI appearance of hemophilic arthropathy included joint effusion or joint haemarthrosis, synovial hypertrophy and haemosiderin deposition. Severe joint disease demonstrated subchondral erosion, cysts or cartilage loss. Denver score was 0 in 10 joints, 1-6 scores in 14 joints, 7-10 scores in 41 joints. There was a positive correlation between Denver score with the course of disease and total number of haemorrhage (r=0.672, 0.770, P<0.05). Conclusion MRI can identify the features of hemophilic arthropathy. The course of disease and total number of haemorrhage are positively correlated with Denver score.
