目的:探讨腹腔镜辅助阴式广泛子宫切除加盆腔淋巴清扫术(LAVRH)在早期宫颈癌的临床效果.方法:采用系统回顾性分析,对2010年2月到2012年3月在我科手术治疗的53例宫颈癌患者进行分析.其中LAVRH组38例为研究组,开腹组15例为对照组.比较两组患者手术时间、术中出血量、手术并发症、术后肠道功能恢复时间、切口愈合情况.结果:两组均未发生严重并发症.手术时间:LAVRH组(268.95±29.69)分钟,开腹组(252±29.57)分钟;出血量:LAVRH组术(229.74±90.75)ml,开腹组(522.66±140.48)ml;术后肠道功能恢复时间:LAVRH组(44.58±9.28)小时,开腹组(57.47±8.98)小时;两组术后尿潴留发生率以腹腔镜组较高,但无显著性差异(P>0.05);两组均达到淋巴清除彻底.结论:腹腔镜辅助阴式广泛子宫切除加盆腔淋巴清扫术具有微创、恢复快、术中出血量少等优点,近期疗效满意,远期效果仍需要大量样本的论证研究.

Objective:To investigate clinical effect of laparoscopy assisted radical vaginal hysterectomy and pelvic lymphadenectomy in early-stage cervical cancer. Methods:We have undertaken a systematic retrospective analysis of 53 patients with cervical cancer from Feb.2010 to Mar. 2012, comparing those presenting at LAVRH surgery (study group, n=38)with those presenting at Laparotomy(control group, n=15). Compared two groups of operation time, intraoperative blood loss, surgical complications, intestinal function recovery time, wound healing, etc. Results:No complications were observed in two groups. Operation time is (268.95±29.69) minutes in study group and (252±29.57) minutes in control group. Intraoperative blood loss is (229.74±90.75) ml in study group and522.66±140.48 ml in control group. Intestinal function recovery time is (44.58±9.28) hours in study group and (57.47±8.98) hours in control group. Incidence of urinary retention in study group is more than in control group, but our statistical data showed that study group had significantly less Incidence of urinary retention compared to control group(P<0.5). Conclusion:Laparoscopy assisted radical vaginal hysterectomy and pelvic lymphadenectomy has the advantages of minimally invasive, less blood loss, faster recovery, etc. Recent results were satisfactory, long-term effects still need a large number of samples of the demonstration study.
