目的探讨大网膜、小网膜及盆腹膜不同组织取材在胃癌腹膜微转移检测中的价值.方法应用实时荧光定量 RT-PCR 检测36例胃癌患者大网膜、小网膜及盆腹膜 CK20mRNA,另以 6例胃癌组织作为阳性对照,6例经腹手术非胃癌患者同类标本作为阴性对照,通过统计学分析研究三种组织取材方式在胃癌腹膜微转移检测中的应用价值.结果36例胃癌患者中,大网膜检测阳性率为41.62%(15/36)、小网膜检测阳性率为13.89%(5/36)、盆腹膜检测阳性率为36.11%(13/36).大网膜检测阳性率高于小网膜和盆腹膜,但与盆腹膜差异无统计学意义.6例胃癌组织检测均为阳性,6例经腹手术非胃癌患者同类标本检测均为阴性.结论大网膜检测胃癌腹膜微转移阳性率高于盆腹膜和小网膜,且取材方便,无副损伤及并发症,可作为组织取材首选方式.

Objective To explore value of great omentum, small omentum and basin peritonum in gastric cancer peritonum micrometastasis detection. Methods Fluorescence quantitative RT-PCR was used to detect CK20mRNA in great omentum small omentum and basin peritonum 0f 36 gastric cancer patients, 6 cancer tissue were detcted as positive comparision and 6 benign disease abdominal surgery patients similar tissue were taken as negative comparision. Stastical method was used to analyze all results. Results In 36 cases, CK20mRNA positive rate of great omentum was 41.62% (15/36), small omentum was 13.89% (5/36) and basin peritonum was 36.11% (13/ 36). The positive rate of great omentum of was higher than small omentum and basin peritonum, but differance between great omentum and basin peritonum had no stastical signfence. And 6 cancer tissue ware all positive while 6 benign disease abdominal surgery patients similar tissue were all negative. Conclusion Great omentum postive rate was higher than small omentum and basin peritonum in detecting gastric cancer peritonum micrometastasis, it was collected conviently and without vice-damage ,so it may be first choice of tissue sellection.
