目的探讨血清同型半胱氨酸与胱抑素 C 在2型糖尿病肾病患者中的变化意义.方法选择我院糖尿病患者共74例,上述患者根据尿白蛋白排泄率分为糖尿病非合并肾病和糖尿病早期肾病组.同时选择同期健康体检者50例,作为对照组.检测所选对象的同型半胱氨酸、胱抑素 C、血肌酐、血尿素氮水平.结果糖尿病非合并肾病组和糖尿病早期肾病组同型半胱氨酸、胱抑素 C、血肌酐、血尿素氮水平均高于对照组,差异有统计学意义(P <0.05);糖尿病早期肾病组同型半胱氨酸及胱抑素 C 水平高于糖尿病非合并肾病组,差异有统计学意义(P <0.05);糖尿病肾病组血肌酐和血尿素氮水平分别与糖尿病非合并肾病组比较,差异无统计学意义(P >0.05).结论了解血清同型半胱氨酸与胱抑素 C 水平有助于糖尿病早期肾病诊断,具有重要的临床意义.

Objective To investigate the clinical significance of the level serum of homocysteine and cystatin C in type 2 diabetic patients with nephropathy. Methods 74 cases of diabetic patients were selected and divided into non diabetic nephropathy and diabetic nephropathy group according to serum albumin excretion rate. 50 cases who carried on physical examination were selected as control group,and they were healthy.Homocysteine, Cystatin C, serum creatinine, blood urea nitrogen level were detected. Results The level of homocysteine, Cystatin C, serum creatinine and blood urea nitrogen in non diabetic nephropathy group and diabetic nephropathy group were higher than those in control group,the difference was statistically significant ( P<0.05 ); The level of homocysteine and cystatin C in diabetic nephropathy group were higher than those in non diabetic nephropathy group, the difference was statistically significant ( P<0.05 ); the level of serum creatinine and blood urea nitrogen in diabetic nephropathy group compared with those in non diabetic nephropathy group, the difference was not statistically significant (P>0.05 ).Conclusion Nnderstanding of serum homocysteine and serum cystatin C levels contribute to the early stage of diabetic nephropathy diagnosis, and which has important clinical significance.
