目的探讨血清胱抑素 C(CysC)用于早期诊断糖尿病肾病(DN)的临床价值.方法采用全自动生化仪、胶乳增强免疫比浊法分别测定100例糖尿病患者和60例健康人员(对照组)的血清 CysC,同时测定血清肌酐(Cr)、血尿素氮(BUN)浓度并与之比较.结果血清 CysC 浓度在2型糖尿病无肾病组与健康对照组比较差异无统计学意义(P>0.05);在糖尿病肾病患者组血清 CysC、BUN、Cr浓度明显高于健康对照组,经 t 检验,差异有统计学意义(P<0.01).糖尿病肾病患者组中血清 CysC、BUN、Cr 阳性率分别为95.7%、65.7%、75.7%,血清 CysC 阳性率与 BUN 和 Cr 比较差异有统计学意义(P<0.01).结论血清 CysC 是动态观测糖尿病肾病患者肾功能的最理想指标,对诊断糖尿病肾病早期肾损伤有重要的临床参考及应用价值.

Objective? To investigate the serum cystatin C ( CysC ) for early diagnosis of diabetic nephropathy ( DN ) and the clinical value. Methods Using automatic biochemical analyzer, latex enhanced immunoturbidimetric assay were measured in 100 cases of diabetes mellitus patients and 60 healthy people ( control group) in the serum CysC, serum creatinine ( Cr ), blood urea nitrogen ( BUN ) concentration and comparison. Results The serum CysC concentrations in type II diabetes without nephropathy group and the healthy control group to compare difference not to have statistical significance (P>0.05);in patients with diabetic nephropathy group serum CysC, BUN, Cr concentration was significantly higher than that of healthy controls, inspection by T, the difference was statistically significant ( P<0.01 ). In patients with diabetic nephropathy group serum CysC, BUN, Cr positive rates were 95.7%, 65.7%, 75.7%, the positive rate of CysC in serum and BUN and Cr had significant difference ( P<0.01 ). Conclusion Serum CysC is a dynamic observation of renal function in patients with diabetic nephropathy is the most ideal index, in the diagnosis of early diabetic nephropathy renal injury have important clinical reference and application value.
