目的?探讨丝裂霉素C脂质体(MMC)复合物预防外伤后关节粘连的作用.方法?实验用兔32只,动物随机分成两组,丝裂霉素C脂质体组(A组)和对照组(B组),所有动物均行右后肢膝关节广泛滑膜切除,克氏针两枚穿越股骨髁及胫骨平台交叉固定,术中A组膝关节内注入丝裂霉素C脂质体复合物,对照组注射生理盐水,术后右后肢管型石膏固定.于术后4周处死,进行肉眼观察,光镜观察,生物力学测定及生物化学分析,并对有关数据进行相应的统计学分析.结果?在大体标本粘连等级方面,A组与B组比较有显著的统计学差异(P<0.05).光镜组织学观察显示A组滑膜上皮细胞增生不明显,纤维结缔组织较少,细胞形态接近正常,基质胶原纤维少.在膝关节活动度方面,A组与B组比较有显著的统计学差异(P<0.01).生化分析结果方面,A组粘连组织中总胶原含量明显低于B组,差异有显著性意义(P<0.01).结论?丝裂霉素组的粘连程度显著轻于对照组,丝裂霉素C脂质体(MMC)复合物可以有效地预防外伤后膝关节粘连.

?Objective?To?study?the?role?of?Mitomycin?C(MMC)?lipid?compound?in?prevention?of?joint?adhesion.?Methods?The?32?New?zealand?white?rabbits?were?divided?randomly?into?two?groups:the?MMC?group?and?the?control?group.?All?animals?underwent?synovectomy?of?right?knee?joint.?Mitomycin?C?lipid?compound?was?inserted?in?the?pouchareas?in?the?experimental?group,whereas?physiological?saline?was?insreted?in?the?control?group.?Leg?was?immobilized?with?plaster?cast?from?toe?to?groin?after?operation.?The?results?were?assessed?by?biomechanics,biochemistry?,histology?and?microscope?microscopical?analysis?in?the?end?of?fourth?weeks?after?operation.?Results?Dense?fibrous?adhension?was?observed?in?the?control?group,whereas?little?adhension?was?found?in?experimental?group(P<0.05).?Histological?evaluation?of?light?microscope?showed?adhesion?of?the?synovial?membrance?of?MMC?group?was?slight,there?was?fewer connective tisue and collagen fiber in cell,and the shape of cell was norma1(P<0.01).The?result?showed?that?the?angular?movement?of?knee?joint?of?MMC?group?was?greater?than?control?group(P<0.01). The total collagen weight of adhension tissue in experimental group was signifiantly less than that of control?group(P<0.01).?Conclusion The results of this experimental study confirm the hypothesis that MMC can prevent or reduce adhesion of knee joint after?injury.
