目的:探讨影响晚期肝门部胆管癌患者内镜支架术后生存时间的主要因素.方法:行内镜治疗的61例晚期肝门部胆管癌患者,选择可能对术后生存时间有影响的10个因素进行单因素分析,通过Cox比例风险模型对其进行多因素分析.结果:单因素分析显示,肿瘤的扩散、胆碱酯酶、白蛋白、碱性磷酸酶、谷丙转氨酶及谷草转氨酶的水平影响患者内镜支架术后的预后(P<0.05),Cox比例风险模型多因素分析显示,肿瘤的扩散、白蛋白及胆碱酯酶的水平是影响预后的独立危险因素.结论:影响晚期肝门部胆管癌患者内镜支架术后的独立危险因素,是肿瘤的扩散、胆碱酯酶及白蛋白水平.

목적:탐토영향만기간문부담관암환자내경지가술후생존시간적주요인소.방법:행내경치료적61례만기간문부담관암환자,선택가능대술후생존시간유영향적10개인소진행단인소분석,통과Cox비례풍험모형대기진행다인소분석.결과:단인소분석현시,종류적확산、담감지매、백단백、감성린산매、곡병전안매급곡초전안매적수평영향환자내경지가술후적예후(P<0.05),Cox비례풍험모형다인소분석현시,종류적확산、백단백급담감지매적수평시영향예후적독립위험인소.결론:영향만기간문부담관암환자내경지가술후적독립위험인소,시종류적확산、담감지매급백단백수평.
Objective To explore the main factors for the survival time after endoscopic treatment of termi?nal hilar cholangiocarcinoma. Methods Sixty-one patients with advanced hilar cholangiocarcinoma received endoscopic therapy. Ten factors that may affect the survival time were selected for univariate analysis, and multi?variate analysis was performed by the Cox proportional hazards model. Results Univariate analysis showed that the proliferation of tumor, cholinesterase, albumin, alkaline phosphatase, bilirubin, alanine aminotransferase and aspartate aminotransferase levels affected the prognosis of advanced hilar cholangiocarcinoma in patients treated with endoscopic stent implantation(P < 0.05), Cox proportional hazard model multivariate analysis showed that proliferation of tumor, albumin and cholinesterase levels were independent risk factors. Conclusion Independent risk factors for advanced hilar cholangiocarcinoma in patients treated with endoscopic stent im?plantation are the proliferation of tumor, cholinesterase, and albumin levels.
