目的探讨老年患者鼻饲后,保持半卧位时间与胃潴留的关系.从而寻求老年患者肠内营养治疗后保持半卧位的最佳时间.方法选择长期鼻饲的老年患者24人,每天每人鼻饲5次,连续观察10天,共计鼻饲1200人次.随机将患者分为对照组(30分钟)8人,实验一组(60分钟)8人,实验二组(90分钟)8人.鼻饲时均将床头抬高30°~45°,鼻饲后保持此体位,分别于30分钟后,60分钟后,90分钟后放低床头(保留胃管患者始终保持床头抬高10°~15°).并于下次鼻饲前回抽胃液,将三组潴留量进行对比,从而得出结论.结果三组患者在鼻饲饮料种类、每次用量、间隔时间、鼻饲温度及速度、浓度均无差异的情况下,因保持鼻饲体位时间长短的不同,从而实验一组的Ⅱ及实验二组的Ⅱ均与对照组胃潴留量对比 P <0.05有统计学差异.结论鼻饲后保持半卧位60分钟即实验一组的方法为最佳方法,此法能有效促进胃肠道的消化与排空,减少胃潴留、胃反流,降低误吸发生的风险.

Objective To discuss the relationship between maintain half lie location time and gastric retention after nasal feeding .Thus to find best time of maintain half lie location after elderly patients with enteral nutrition therapy .Methods Choosing 24 elderly patients with nasal feeding .Nasal feeding five times per day ,continuously observe 10 days .A total of 1200 person-time have nasal feeding .Patients were divided into control group (30 minutes ,8 patients) and experimental first group (60 minutes ,8 patients) ,experimental second group (90 minutes ,8 patients) randomly .Elevate head of bed 30 - 45° ,and keeping this position after nasal feeding .Lower the head of bed after 30 minutes ,60 minutes ,90 minutes (Keep stomach tube patients always keep the head of bed up 10 - 15°) .Withdraw gastric juice before the next nasal feeding .Three groups of retention quantity are compared .Results Three groups of elderly nasal feeding patients in the same conditions of nasal feeding drink types ,each time the amount ,spacing in-terval ,nasal feeding temperature ,speed and concentration and different time length have statistical y significant ( P < 0 .05) .Conclusion The method of experimental first group is the best method .This method can effectively promote gastrointestinal digestion and emptying ,reduce gastric retention , stomach reverse flow ,reduce the risk of aspiration happen .
