目的研究妊娠合并失血性休克患者的一般资料和休克原因动态变化数据,为临床和科研工作提供依据.方法统计北京妇产医院2009年-2011年收治的妊娠合并失血性休克患者的年龄、休克发生时的产妇孕周及导致失血性休克发生的原因.比较各项指标的年度动态变化有无统计学显著差异(采用 SPSS16.0统计软件进行统计分析).结果按年度划分,患者的平均年龄分别为32.68±4.58岁、32.17±4.81岁、30.37±3.89岁,年度间无统计学差异( P < 0.05);休克发生时孕产妇的平均孕周分别为36.37±4.33周、35.00 ± 5.15周、35.71±4.90周,年度间无统计学差异( P < 0.05);导致失血性休克发生的原因包括:产后宫缩乏力(年发生率分别为13/31、9/20、8/19);前置胎盘和胎盘植入(年发生率分别为8/31、6/20、6/19)、子宫阴道会阴破裂(年发生率分别为4/31、3/20、2/19)、羊水栓塞合并 DIC(年发生率分别为2/31、0/20、0/19)、部分胎盘残留(年发生率分别为2/31、1/20、2/19)、其它不明原因(年发生率分别为2/31、1/20、1/19),对上述各指标进行统计学分析,年度间均无统计学显著差异( P < 0.05).结论妊娠合并失血性休克孕产妇的年龄、休克发生时的平均孕周及导致失血性休克发生的原因无明显年度变化.

Objective To investigate dynamic change data of general data and shock reason of pregnancy inpatients with hemorrhagic shock in Bei -jing obstetrics and gynecology hospital from 2009 to 2011 ,and to provide a basis for clinical and scientific research work .Methods Analyzing the age , gestational ? weeks and the cause of hemorrhagic shock of pregnancy inpatients with hemorrhagic shock in Beijing obstetrics and gynecology hospital from 2009 to 2011 .Comparing various index of the annual dynamic change to see if it have statistical y significant differences in three years (Using SPSS16 .0 statistical software for statistical analysis ) .Results The average age of pregnancy inpatients with hemorrhagic shock from 2009 to 2011 were 32 .68 ± 4 .58 years old 、32 .17 ± 4 .81 years old 、30 .37 ± 3 .89 years old respectively .No statistic difference among three years( P < 0 .05) ;The aver-age gestational ? weeks when the hemorrhagic shock occurred were 36 .37 ± 4 .33 weeks 、35 .00 ± 5 .15 weeks 、35 .71 ± 4 .90 weeks .No statistic differ-ence among three years( P < 0 .05) .The causes of hemorrhagic shock include postpartum uterine inertia (incidence rate were 13/31 ,9/20 ,8/19 re-spectively ) ;the placenta praevia and placenta increta (incidence rate were 8/31 ,6/20 ,6/19 respectively ) ;the rupture of uterus ,vagina and perineum (incidence rate were 4/31 ,3/20 ,2/19 respectively ) ;the amniotic fluid embolism complicated DIC (incidence rate were 2/31 ,0/20 ,0/19 respectively ) ;residue of placenta(incidence rate were 2/31 ,1/20 ,2/19 respectively ) ;the other unknown reasons(incidence rate were 2/31 、1/20 、1/19 respective-ly) .Al indexes above have no statistical significance among the three years ( P < 0 .05) .Conclusion The average age ,average gestational weeks of pregnancy inpatients with hemorrhagic shock and the causes of the hemorrhagic shock have no obvious annual change from 2009 to 2011 in Beijing ma-ternity hospital .
