目的分析漩涡参数涡量(Q)与漩涡圈数(N)、涡强(Q/S)与漩涡最大向量速度(Vmax )之间的相关性,探讨可以反映左心室舒张功能的漩涡参数.方法选取健康对照者(健康对照组)40名和舒张功能早期减低患者(病例组)40例,应用血流速度向量和流线(VFM)技术观察2组受检者心动周期各时相左心室漩涡形成规律,比较漩涡参数:Q、面积(S)、Q/S、Vmax、N、涡量比(RQ )、涡强比(RQ/S ).结果病例组与健康对照组受检者左心室漩涡衍变规律相似,均表现为二尖瓣下小对涡和左心室内大涡交替出现.健康对照组:舒张早期Q大于舒张晚期Q[(33.62±24.38)cm 2/s vs (14.65±10.71)cm 2/s],差异有统计学意义(t=2.849,P<0.05),舒张早期Q/S大于舒张晚期Q/S [(26.00±8.80)s-1 vs (15.13±7.03)s-1],差异有统计学意义(t=3.859,P<0.05),舒张早期Vmax大于舒张晚期Vmax [(79.32±32.65) cm/s vs (33.99±9.22) cm/s],差异有统计学意义(t=2.049, P<0.05);舒张早期N大于舒张晚期N[(6.67±3.16)n vs (4.08±2.51)n],差异有统计学意义(t=1.949, P<0.05).病例组:舒张早期Q小于舒张晚期Q[(16.57±12.12)cm 2/s vs (35.16±13.04)cm 2/s],差异有统计学意义(t=2.764,P<0.05);舒张早期Q/S小于舒张晚期Q/S[(16.11±6.19)s-1 vs (27.58±7.66)s-1],差异有统计学意义(t=3.112,P<0.05);舒张早期Vmax小于舒张晚期Vmax [(39.89±9.01)cm/s vs (59.12±16.9) cm/s],差异有统计学意义( t =3.214,P <0.05);舒张早期 N 小于舒张晚期N[(2.81±1.21)n vs (7.07±3.16)n],差异有统计学意义(t=1.842,P<0.05).病例组和对照组Q与N显著相关(r=0.881和0.821,P=0.000和0.001),Q/S与Vmax显著相关(r=0.901和0.871, P均=0.000),RQ/S与E/A比值显著相关(r=0.964和0.851,P均=0.000).结论 VFM技术可以反映舒张功能早期减低患者左心室内涡流的特点,新的漩涡参数Q、Q/s可以代替原参数Vmax、N,RQ/s在一定程度上可以反映左心室舒张功能.

Objective To discuss the correlation of vortex parameters Q (maximum flow volume of vortex,Q) and N(number of vortex,N),Q/S(vortex intensity,Q/S)and Vmax(maximum vector velocity, Vmax ) ,and discuss their value in evaluating the left ventricular diastolic function .Methods Forty normal adults and forty patients with early reduction of diastolic function were recruited in control and study group , VFM(vortex flow mapping)technique was applied to observe the vortex formation laws of two groups ,vortex parameters:Q,area(S),Q/S,Vmax ,N,and the ratio parameters of maximum flow volume of vortex (RQ ),and the ratio parameters vortex intensity(RQ/S )were compared.Results Vortex formation laws in the case group were similar to the control group,the twins eddys under the mitral valve and big eddy showed alternatively in the left chamber.In the control group,Q in early diastolic was greater than that in late diastolic[(33.62 ±24.38)cm 2/s vs (14.65 ±10.71)cm 2/s](t=2.849,P<0.05),as well as Q/S[(26.00 ±8.80)s-1 vs (15.13 ±7.03)s-1 ] (t=3.859,P<0.05),Vmax [(79.32 ±32.65)cm/s vs (33.99 ±9.22)cm/s](t=2.049,P<0.05),and in N[(6.67 ±3.16) n vs (4.08 ±2.51) n] (t=1.949,P <0.05),the difference were all statistically significant,while in the study group,the result was opposite,Q in early diastolic was less than that in late diastolic[(16.57 ±12.12) cm2/s vs (35.16 ±13.04) cm2/s],the difference was statistically significant (t=2.764,P<0.05),Q/S[(16.11 ±6.19) s-1 vs (27.58 ±7.66) s-1 ] (t=3.112,P<0.05),Vmax [(39.89 ±9.01)cm/s vs (59.12 ±16.9) cm/s] ( t =3.214,P <0.05) and N [(2.81 ±1.21) n vs (7.07 ±3.16)n](t=1.842,P<0.05),the difference were all statistically significant.Significantly correlation between the Q and N(r=0.881 and 0.821,P=0.000 and 0.001);Q/S and Vmax (r=0.901 and 0.871,all P=0.000);the ratio of RQ/S and E/A(r=0.964 and 0.851,all P=0.000)was found in both the control and study groups respectively.Conclusions VFM can reflect vortex characteristics in the left ventricular for patients with reduced early diastolic function .New vortex parameter Q,Q/S could potentially replace the original parametersV max ,N.RQ/S is much more valuable and can reflect left ventricular diastolic function .
