目的观察舒芬太尼静脉自控镇痛对肺癌手术后患者疼痛和血清IL-6、IL-10水平的影响.方法选择我院收治的择期肺癌手术患者60例,ASAⅠ~Ⅱ级,随机分为三组,每组20例,包括生理盐水对照组(C组)、芬太尼组(F组)和舒芬太尼组(S组),观察并比较三组患者术后血清IL-6和IL-10的变化及VAS评分.结果与C组相比,F和S组患者的VAS评分、血清IL-6和IL-10水平均显著降低(P<0.05).与F组相比,S组患者血清IL-6和IL-10水平和VAS评分亦显著降低(P<0.05).结论舒芬太尼静脉自控镇痛用于肺癌手术后患者的镇痛效果更好,且能有效地维持细胞因子的平衡,缓解术后的免疫损伤.

목적관찰서분태니정맥자공진통대폐암수술후환자동통화혈청IL-6、IL-10수평적영향.방법선택아원수치적택기폐암수술환자60례,ASAⅠ~Ⅱ급,수궤분위삼조,매조20례,포괄생리염수대조조(C조)、분태니조(F조)화서분태니조(S조),관찰병비교삼조환자술후혈청IL-6화IL-10적변화급VAS평분.결과여C조상비,F화S조환자적VAS평분、혈청IL-6화IL-10수평균현저강저(P<0.05).여F조상비,S조환자혈청IL-6화IL-10수평화VAS평분역현저강저(P<0.05).결론서분태니정맥자공진통용우폐암수술후환자적진통효과경호,차능유효지유지세포인자적평형,완해술후적면역손상.
Objective To evaluate the effects of the controlled intravenous analgesia by sufentanil on plasma IL-6 and IL-10 levels on patients who suffered lung cancer operation. Methods Sixty lung cancer surgery patients (ASAⅠ-Ⅱgrade) were selected and divided randomly into three groups (n=20):Saline control group (group C), Fentanyl group (group F) and sufentanil group (group S). The visual analog scale (VAS) and serum concentrations of IL-6 and IL-10 were measured and compared between the three groups. Results Compared with group C, VAS scores and the levels of IL-6 and IL-10 on av-erage significantly decreased in group F and group S (P<0.05). Compared with group F, the serum levels of IL-6 and IL-10 and VAS significantly decreased in group S (P<0.05). Conclusion Controlled intravenous analgesia by Sufentanil is more effective than Fentanil in the treatment of patients after lung cancer operation. It could effectively maintain the balance of cy-tokines and relieve the immunologic injury post operation.
