目的:观察瑞芬太尼联合异丙酚在纤维支气管镜检查的临床应用.方法:抽取进行纤维支气管镜检查的患者80例,随机分为两组,试验组(瑞芬太尼+异丙酚)和对照组(利多卡因),每组40例.试验组(RP组)给予静脉注射异丙酚1 mg/kg持续60 s,随后用瑞芬太尼微泵以0.1μg/(kg·min)持续输注,待患者入睡、呼之不应、睫毛反射消失后,开始纤维支气管镜检查;对照组(L组)给予2%利多卡因10 ml表面麻醉.结果:试验组麻醉起效快,血流动力学稳定,组内检查前中后比较变化不大(P>0.05),对照组入镜后血压、心率较检查前明显升高,呼吸频率增快(P<0.05),对照组呛咳和躁动不安发生率明显高于试验组(P<0.01),患者满意度低于试验组(P<0.01).两组患者在供氧状态下SpO2均≥98%.结论:瑞芬太尼联合异丙酚用于纤维支气管镜检查安全舒适,可控性好,值得临床推广.
목적:관찰서분태니연합이병분재섬유지기관경검사적림상응용.방법:추취진행섬유지기관경검사적환자80례,수궤분위량조,시험조(서분태니+이병분)화대조조(리다잡인),매조40례.시험조(RP조)급여정맥주사이병분1 mg/kg지속60 s,수후용서분태니미빙이0.1μg/(kg·min)지속수주,대환자입수、호지불응、첩모반사소실후,개시섬유지기관경검사;대조조(L조)급여2%리다잡인10 ml표면마취.결과:시험조마취기효쾌,혈류동역학은정,조내검사전중후비교변화불대(P>0.05),대조조입경후혈압、심솔교검사전명현승고,호흡빈솔증쾌(P<0.05),대조조창해화조동불안발생솔명현고우시험조(P<0.01),환자만의도저우시험조(P<0.01).량조환자재공양상태하SpO2균≥98%.결론:서분태니연합이병분용우섬유지기관경검사안전서괄,가공성호,치득림상추엄.
Objective:To evaluate the clinical application of remifentanil combing with propofol to fiberoptic bronchoscopy examination. Methods:80 patients undergoing check of fiberoptic bronchoscope were randomly divided into 2 groups with 40 patients in each. Group RP was given remifentanil and propofol, group L only received topical anesthesia of lidocaine(2%).Hemodynamic date (MAP,HR,RR and SpO2) were recorded.Observe adverse effects and satisfaction of patient.Results:Group RP took effect very quickly.The MAP and HR was more stable in the group RP.The bucking and the agitation of group RP were lower than that in group L(P<0.01).Patient’s satisfaction of group RP was higher than the other(P<0.01).The SpO2 were more than 98%with the oxgen supporting in every group. Conclusion:It was safe and effective using remifentanil combing with propofol to fiberoptic bronchoscopy examination. It had the advantages of good control,less hemodynamic effect and can be used clinically.
