Activated carbon nanoparticles%5 fluorouracil%Middle-low rectal cancer
目的探讨5-氟尿嘧啶(5-Fu)纳米炭示踪剂在中低位直肠癌手术中的应用价值.方法选取中低位直肠癌手术患者53例,随机分为观察组32例和对照组21例.观察组术前应用5-氟尿嘧啶纳米炭淋巴示踪剂;对照组常规行直肠癌根治术.比较两组病理标本所检出的淋巴结数量、转移淋巴结癌细胞凋亡指数、术后淋巴化疗所引起的毒副作用.结果观察组平均淋巴结数量(16.23±5.50)个,明显高于对照组的(11.28±6.35)个(P<0.05).观察组检出微小淋巴结(直径≤2 mm)的比例为27.98%(141/504),对照组微小淋巴结的比例为18.17%(43/238),观察组检获微小淋巴结明显多于对照组(P<0.05).观察组淋巴结转移细胞凋亡指数为(33.58±3.27)%;对照组转移淋巴结中细胞凋亡指数为(6.11±2.52)%.观察组转移淋巴结中细胞凋亡率明显高于对照组,两组间差异有统计学意义(P<0.05).没有发现由于使用5-氟尿嘧啶纳米炭淋巴示踪剂所引起的毒副作用.结论使用5-氟尿嘧啶纳米炭淋巴示踪剂有利于中低位直肠癌的手术及综合治疗.

Objective To evaluate the outcomes of activated carbon nanoparticles carrying 5-fluorouracil (5-Fu) in radical operation of middle-low rectal cancer. Methods Fifty-three patients with middle-low rectal cancer were enrolled and randomLy divided into two groups in this study . The activated carbon nanoparticles carrying 5-Fu were injected around the tumor regions before surgical resection in 32 patients (group A), Which were not in other 21 patients underwent the standard radical operation as the control( group B). The black-stained sentinel lymph nodes were observed. The number of lymph nodes in pathologic samples, the ratio of tiny lymph node (diameter 2 mm or less) and the apoptosis index of cancer cells in metastatic lymph nodes were compared between the two groups. Results The mean number of lymph nodes in pathological samples was significantly higher in group A than that in group B (16.23±5.50 and 11.28±6.35,P<0.05). The ratio of tiny lymph node in group A was more than that in group B (27.98% vs 18.17%,P<0.05). The apoptosis index of cancer cells in metastatic lymph nodes was significantly higher in group A than that in group B( (33.58±3.27)%and (6.11±2.52)%,P<0.05). The side effects were not found in group A. Conclusion Application of activated carbon nanoparticles carrying 5-Fu may be an useful adjuvant therapy in radical operation of the patients with middle-low rectal cancer.
