目的以病理活检为基础,对TCT及阴道镜检查在宫颈癌筛查中的价值进行评价.方法对我院妇科门诊行TCT检查的3451例患者中,细胞学筛查阳性559例及细胞学阴性而临床高度可疑的82例患者共641例同时行阴道镜下宫颈活检.结果 TCT诊断CIN与宫颈癌的敏感度0.7474,特异度0.8076,阳性预测值0.6277,阴性预测值0.8805,符合率0.7894.阴道镜诊断CIN与宫颈癌的灵敏度为0.9588,特异度为0.8904,阳性预测值为0.7915,阴性预测值为0.9803,符合率0.9111.结论 TCT与阴道镜检查均可作为宫颈癌筛查手段,二者有互补,但阴道镜检查结果灵敏度、特异度及符合率均较TCT高.

@@@@Objective Biopsy results as the standard to evaluate the TCT and colposcopy in cervical screening value. Methods We have screened out of 641 cases to colposcopy and to biopsy simultaneously (including 559 cases of positive cytology and 82 patients with clinically highly suspicious and cytology negative patients). Results TCT diagnosis of CIN and cervical cancer sensitivity of 0.7474, specificity of 0.8076, positive predictive value of 0.6277, negative predictive value of 0.8805, in line with rate 0.7894. Colposcopic diagnosis of CIN and cervical cancer sensitivity of 0.9588 and specificity of 0.8904, 0.7915 positive predictive value, negative predictive value of 0.9803, in line with rate 0.9111. Conclusion TCT and colposcopy can be used as means of cervical cancer screening, but the results of colposcopy sensitivity, specificity and high compliance rate than the TCT.
