目的应用斑点追踪成像(STI)评价原发性高血压患者左心室扭转及解旋运动的变化,并探讨其临床意义.方法选取原发性高血压患者62例,其中非LVH患者32例,轻度LVH患者30例,另选取健康志愿者30例为正常对照组.应用斑点追踪成像计算左心室峰值扭转角度(Ptw),收缩期末扭转角度(AVCtw),等容舒张期末扭转角度(MVOtw),解旋率(UntwR).结果与正常对照组相比,高血压组的Ptw、AVCtw、MVOtw均增大(P<0.05),三组间的UntwR逐渐减小(P<0.05).结论原发性高血压患者左心室扭转运动增大,解旋运动随着心肌的肥厚而减低,STI为原发性高血压患者机械力学的评价提供了一个新途径.

목적응용반점추종성상(STI)평개원발성고혈압환자좌심실뉴전급해선운동적변화,병탐토기림상의의.방법선취원발성고혈압환자62례,기중비LVH환자32례,경도LVH환자30례,령선취건강지원자30례위정상대조조.응용반점추종성상계산좌심실봉치뉴전각도(Ptw),수축기말뉴전각도(AVCtw),등용서장기말뉴전각도(MVOtw),해선솔(UntwR).결과여정상대조조상비,고혈압조적Ptw、AVCtw、MVOtw균증대(P<0.05),삼조간적UntwR축점감소(P<0.05).결론원발성고혈압환자좌심실뉴전운동증대,해선운동수착심기적비후이감저,STI위원발성고혈압환자궤계역학적평개제공료일개신도경.
Objective The aim of this study was to evaluate left ventricular twisting and untwisting in primary hypertension patients, and to investigate its clinical significance. Methods We selected 62 subjects with primary hypertension, including 32 non-hypertrophic patients and 30 mild hypertrophic patients, we also selected 32 healthy controls. We measured peak basal rotation, peak apical rotation in basal and apical short-axis views with speckle tracking imaging, and calculated peak left ventricular torsion(Ptw), twist at aortic valve closure(AVCtw), twist at mitral valve opening(MVOtw)and untwisting rate(UntwR). Results Ptw, AVCtw and MVOtw was significantly higher in primary hypertension patients compared with control group (P<0.05), and UntwR reduced in primary hypertensive patients (P<0.05). Conclusion Left ventricular twisting is higher in primary hypertensive patients, and untwisting declined. Speckle tracking imaging may provide a new approach for the evaluation of the mechanics of patients with hypertension.
