目的探讨腹腔镜胃癌根治术患者外周血 CK19mRNA的变化及意义.方法采用荧光定量聚合酶链反应(FQ-PCR)技术检测CK19mRNA在腹腔镜组(14例)及开腹组(37例)胃癌根治术外周血的表达情况.结果两组间仅手术时间差异有统计学意义(P<0.01),术中失血量及清除淋巴结数的差异均无统计学意义(均P>0.05).腹腔镜组患者手术前后CK19mRNA的表达无明显变化(P>0.05).开腹组患者手术前后CK19mRNA的表达差异有统计学意义(P<0.05).开腹组术后CK19mRNA阳性表达患者明显多于腹腔镜组,差异有统计学意义(P<0.05).结论与开腹手术相比,腹腔镜胃癌根治术可以降低静脉血CK19mR-NA的阳性率,减少术后肿瘤血循微转移的危险性.

Objective To investigate the expression of CK19 mRNA in patients with gastric cancer undergoing laparo-scopic radical resection and its significance. Methods The expression of peripheral blood CK19mRNA was detected by fluo-rescence quantitative polymerase chain reaction (FQ-PCR) technology in patients with gastric cancer undergoing laparoscopic radical resection(laparoscopy group, n=14) or laparotomy resection(laparotomy group, n=37). Results The operation time in la-paroscopy group was shorter than that in laparotomy group (P<0.01), while there were no significant differences in e intraopera-tive blood loss and number of removed lymph node between two groups (P>0.05). There was no difference in expression of CK19mRNA before and after operation in laparoscopy group (P>0.05), while there was statistical y significant in laparotomy group (P<0.05). The rate of positive CK19mRNA expression in laparotomy group after operation was higher than that in laparo-scopic group (P<0.05). Conclusion Compared to laparotomy, laparoscopic radical resection of gastric cancer can reduce the positive rate of CK19mRNA expression in peripheral blood and the risk of tumor micrometastasis after operation.
