目的探讨超声造影联合超声弹性成像检测对甲状腺癌的诊断价值.方法选取甲状腺结节患者158例(共282个结节),均行单纯超声造影检查以及超声造影联合超声弹性成像检查,以病理结果为“金标准”,计算并比较单纯超声造影与超声造影联合超声弹性成像诊断甲状腺癌的敏感性、特异性及准确性.结果病理检查结果示良性结节240个,恶性结节42个.单纯超声造影诊断甲状腺癌的敏感性为73.8%、特异性为93.7%、准确性为90.7%.超声造影联合弹性成像诊断的敏感性为85.7%、特异性为96.6%、准确性为95.0%.超声造影联合弹性成像诊断准确性明显高于超声造影,差异有统计学意义(P<0.05).结论超声造影联合弹性成像可以提高甲状腺结节良、恶性诊断的准确性.

Objective To investigate the value of contrast-enhanced ultrasonography combined with ultrasound elasticity imaging in diagnosis of thyroid carcinoma. Methods One hundred and fifty-eight patients with 282 thyroid nodules were exam-ined by both contrast enhanced ultrasonography and elastography. The pathological diagnosis was considered as the gold stan-dard, the sensitivity, specificity and accuracy of examinations in diagnosis of thyroid carcinoma were assessed. Results The sensitivity, specificity and accuracy of contrast enhanced ultrasonography in diagnosis of thyroid carcinoma were 73.8%, 93.7%and 90.7%respectively;those of combined examination were 85.7%, 96.6%and 95.0%respectively. There was significant differ-ence in the accuracy of diagnosis between two groups (P<0.05), but there were no differences in sensitivity, specificity, positive predictive value and negative predictive value between two groups. Conclusion Contrast enhanced ultrasonography combined with elastography imaging can elevate the accuracy in diagnosis of thyroid carcinoma for patients with thyroid nodules.
