医保支付制度改革是我国医改的重要突破口.提出一种新的医保总额预付制方案模型.该模型统筹区域医疗资源,通过在医保基金分配中赋予各类资源不同权重,推动医疗机构为调整医疗资源供给结构而主动引导患者需求的合理流动,从而实现医疗资源均衡利用和缓解看病贵、看病难问题.分析了模型在实施过程中可能发生的典型问题并给出对策建议.

The reform of the medicare payment system is an important breach of Chinese medical reform. A new global budget model is proposed. This model plans regional medical resources as a whole and each type of resource gets its weight which will decide its role in health insurance fund allocation. Medical institutions will be motivated by these weights to adjust their medical resources supply structures and patients will be guided by these institutions to flow to fit those new structures. The use of regional medical resources will be balanced and both the problems of expensive medical treatment and difficulty in seeing doctors will be alleviated. Some typical problems which may occur in the implementation process have been analyzed and some corresponding suggestions have been given.
