目的:对甲胎蛋白(AFP)酶联免疫吸附试验法与甲胎蛋白化学发光法两种检测方法的敏感性和特异性进行对比,评价两种方法在正常人群中甲胎蛋白检测的应用效果.方法:用酶联免疫吸附试验法与化学发光法同时对2000份体检血清标本甲胎蛋白进行检测.结果:用酶联免疫吸附试验法检测出20例甲胎蛋白阳性标本,用化学发光法检测甲胎蛋白21例高于正常范围,化学发光法检出AFP高于正常范围的21例中,阳性率1.05%,经确证AFP高于正常范围的21例,均为阳性;酶联法检出20例阳性,阳性率1.15%,1例经确证为阴性,特异性为95%.两种方法同为阳性的20例标本,经确证均为阳性.结论:在正常体检甲胎蛋白人群中,酶联免疫吸附试验法与化学发光法检测AFP敏感性差别不大,而特异性差别较大,甲胎蛋白酶联免疫吸附试验法更适合于血液的甲胎蛋白筛查.甲胎蛋白筛查阳性的标本再用化学发光法定量检测AFP,可保证排除AFP假阳性.

@@@@Objective:To study the comparability of A-fetoprote in (AFP) the sensitivity and specificity of detection obtained with ELISA and chemilum inescence immunoassay detection methods.Method:The 2000 serum samples of physical examination of AFP used ELISA and chemiluminescence test method detection.Result:17 cases of AFP by ELISA assay positive specimens,chemiluminescence detection of AFP above the normal range in 21 cases,positivity of 1.05%.21 cases were confirmed by positivity.Positive was detected in 17cases with ELISA assay,1 cases were confirmed by negative,the specificity was 95%.Two methods for 16cases with positive,confirmed by positive.Conclusion:In normal alpha-fetoprotein populations,detection sensitivity of AFP with little difference between chemiluminescence and ELISA assay,and specific differences,alpha-fetoprotein alpha-fetoprotei with chemiluminescence is more suitable for blood enzyme linked immunosorbent assay screening.Alpha-fetoprotein and chemical screening specimens positive luminescence method for quantitative detection of AFP,can ensure the elimination of false positive AFP.
